2. The Aims State Focus: Big Aim: Small Aim:
Reduce psychiatric inpatient readmission rates (30 days and 180 days)
Big Aim:
Increase the involvement ACSS clients have regarding their discharge from a psychiatric hospital.
Small Aim:
75% of clients will identify a post-discharge need to their social worker, via telephone, during their hospital stay.

4. The Results
For two months, ACSS had 1 hospitalization. Had to relook at the need for the big-aim and small aim.
Decided to focus NIATx principals on another change project.

5. Next Steps Next 3 to 6 months: Two options:
Another Change Project within ACSS OR
Combine additional units within Behavioral Health for a broader change.
