4 Vision All students who are receiving Early Intervention Services across Nelson, Marlborough, Golden Bay and Westland receive effective support from educators and service providers.

5 Transition to School from MOE Early Intervention
MOE Early Intervention Client (at least 3 months prior to starting school) MOE EI teacher contact School and EI team leader(Fiona) RTLB from EI team assigned Transition meetings organised Support plan developed

6 Transition carefully planned Child starts school

7 Options for first 6 weeks support
MOE transition support RTLB transition support MOE/RTLB co-working transition

8 4/6 week Review Successful transition – no service required
MOE/RTLB Co-work case MOE case (Behaviour) RTLB case – school complete new referral for RTLB service stating concerns and positive outcome desired.

9 MOE Behaviour support and Communication Support
Educational Psychologist will support transition to school – may request support from RTLB but Ed Psych can stay involved with student at school so no need to transition to RTLB role. Speech and Language Therapist – same as above. They can continue to support children as they transition to school and can remain involved in the longer term EI Teachers will discontinue support after students starts school – length of time can be variable

11 Service so far in 2016 39 students supported or in the process of being supported. Across 19 schools 19 students still to start school 20 started 2 students have received ORS funding. 6 discontinued from service 10 RTLB continued support 2 referred to other services
