Mobile Outreach Van

Supporting the health of the homeless and migrants in the community including:
Department of Human Services, Guadalupe Shelter, Lincoln Park, Plaza Del Milagro Salvation Army, Weld County Food Bank, local farms, and many more locations.
Outreach services started in 1973; the mobile van began operating in 2004.
More than 1,400 patients received care in 2014.
