3 Planned Time Off at FAVARH
FAVARH encourages employees to plan time off to: (1) Maintain health, balance, and energy. (2) Ensure that FAVARH maintains adequate levels of staffing for successful programs and business operations. FAVARH encourages employees to plan time off to maintain health, balance, and energy, and to ensure that the Agency maintains adequate levels of staffing for successful program and business operations. Punctual and regular attendance is an essential responsibility of each employee. Employees are expected to report to work as scheduled, on time, and prepared to start work. When an employee is absent, others must perform the work, which diminishes the smooth functioning of the team. Employees are expected to remain at work for their entire work schedule, unless work is not available and/or are asked to clock out or for reasons protected under the Connecticut Paid Sick Leave (PSL) as follows: Care, treatment, or preventative medical care for illness, injuries or health conditions of the employee, the employee’s spouse, or the employee’s minor or disabled children. Medical treatment or counseling, victim services, relocating, or participation in legal proceedings when the employee is a victim of family violence or sexual assault.

4 How do I Plan for Time Off?
Punctual and regular attendance is an essential responsibility of each employee. Employees are expected to report to work as scheduled, on time, and prepared to start work. Employees must notify their supervisor of the need to use PTO as soon as practicable. Employees should provide at least 7 days notice. Employees are encouraged, recognized, and rewarded to achieve Perfect Attendance. The purpose of this policy is to promote the efficient operation of the company and minimize unscheduled absences. Employees must notify their supervisor of the need to use PTO as soon as practicable. Where the need is foreseeable (such as vacation and leisure purposes), employees should provide at least 7 days notice. Supervisors shall approve PTO within 48 hours.

6 What is Perfect Attendance?
NO FLMA and NO PSL NO UNPLANNED TIME OFF 7 DAYS ADVANCED NOTICE Perfect Attendance is the adherence to the FAVARH Attendance Policy which requires 7 days advanced notice for PTO requests with supervisor approval AND no unplanned time off, no FLMA, and no PSL days.

7 Benefits of Perfect Attendance
I had Perfect Attendance this quarter. I am a Direct Support Professional. I am eligible for Rewards & Incentives. Eligible employees will receive Rewards & Incentives for achieving Perfect Attendance. Eligible employees will receive Rewards and Incentives for achieving Perfect Attendance. To be eligible for Rewards and Incentives: (1) The employee must achieve Perfect Attendance each quarter. (2) The employee must be an Instructor/Direct Support Professional.

8 Rewards and Incentives
Employees will receive consistent feedback from managers on their performance. Positive Feedback Managers will acknowledge, recognize, and comment on employee strengths and outstanding work efforts. Acknowledgement Each location will have a bulletin board to post awards, certificates, thank you cards, and other displays of excellent work efforts. Wall of Honor The main office will have a Video Presentation on display for the public to highlight outstanding employees. High Honors Presentation Positive Feedback: Employees will receive positive feedback and comments from managers on a regular and consistent basis. Acknowledgement: Managers will acknowledge, recognize, and comment on employee efforts on a regular and consistent basis. Wall of Honor: Each location will have a bulletin board to post awards, certificates, thank you cards, and other displays of excellent work efforts. Managers and employees can choose what they would like to place on the Wall of Honor to best recognize and showcase outstanding employees. High Honors Presentation: The main office will have PowerPoint Video Presentation on display for the public. The presentation will showcase employees of the month, departments with highest number of outstanding employee performances and attendance records, and other positive or outstanding employee/company work efforts.

9 Rewards and Incentives
Quarterly Rewards: Team Breakfast/Lunch: The team with the best attendance will receive breakfast or lunch from FAVARH attended by the Executive Director. Individual Gift Cards: Gas and Visa gift cards will be given to each employee with perfect attendance. Yearly Rewards: 100% PTO Payback: Direct Support Professionals will be paid for all unused PTO at the end of the fiscal year July 1 to June 30. Dedication Club: Employees with perfect attendance for the entire year will become members of the Dedication Club. They will receive lunch/dinner with the Executive Director, the Directors, and other Club Members. Those who attend will be entered in a raffle for a chance to win a prize.
