17. Separate Athletic Physical Forms are Required
If your child wants to participate in any athletic program at Southern this year, you must have the sports physical form completed before tryouts. The athletic physical form can be found in the front office and on our school webpage. You also must pay for athletic insurance through the school/Fayette County.
