Case Manager + Employment Specialist = Your Team Barrier remediation for families. Have conversations. Use the Toolkit. Connect early. Share invites, share meetings, share notes, share updates, SHARE! Check in biweekly, and, really, whenever you need to.

6 Successes: Data tells the story
Twice as many clients gained two jobs in both 2014 and 2015 as the total # of clients who gained jobs in 2013.

Old model was ineffective Referral process slowed us down Team work makes the dream work WorkFirst and Housing First go hand in hand Funding can be challenging, but is feasible!
