(Group 1: N=30) vs. Retrospective pts. (Group 2: N=130) EAU guidelines follow-up scheme WBC, PLT, N, L counts. (NLR = N / L). Tumor Characteristics Compared : Grade, Stage, Focality, Site, Size. Endpoints: Recurrence at 1st follow-up Multiple recurrences during follow-up Progression (Grade-Stage)

Prospective evaluation of preoperative Neutrophil / Lymphocyte ratio as a prognosticator in upper tract urothelial carcinoma (UTUC) patients treated conservatively Mean NLR value: 2.90 ± 1.05 in Group 1 vs ± 1.92 in Group 2 (p=0.9). Higher NLR values in: ≥pT1 (p=0.03 and p=0.0001) HG (p=0.03 and p=0.0009) Multifocal (p=0.01 and p=0.028) >1 cm tumor (p=0.04 and p=0.0001) NLR>3: Rec. at 1st FU: p=0.04 (HR5.3) + p=0.007 (HR2.9) Multiple Recs. in FU: p=0.02 (HR7.3) + p=0.006 (HR1.5) Progression: p=0.04 (HR5) only in Group 2 In UTUC patients selected for renal conservation therapy high baseline NLR may improve individualized care by identifying those who may need more frequent endoscopic follow-up, and those who should have lower thresholds to conversion to more aggressive surgical strategies.
