Vehicle Identification – Printed or affixed on exterior of vehicle, legible and visable, in a typeface 3 inches high or larger.
Required information remains the same
Business Name
Address, phone number or USDOT census number

Subrule (3) if drift occurs, notify residents of affected area.

Replace with required notification information:
Name, address and phone number of a person who may be contacted
Person is responsible for supplying information concerning the application
