1 SCAS referrals for Non-Conveyed Fallers- Guidance for CCT’s on managing
RED for patients: at risk of admission in 24 hours with 2 or more falls in last week With recent symptoms of syncope/blackout Who cause serious concern to crew – likely to fall again Crew will leave a telephone message with patient details on team phone requesting urgent assessment Electronic referral follows via NHS.net mailbox CCT- Triage and arrange visit to assess within 24 hours AMBER for patients: With 2 or more falls in past 6 months Who have reversible risk factors for falling – preventable re-fallers Referral via NHS.net mailbox CCT - should triage and assess patient GREEN for patients: With an explained fall or first fall - low risk Where the crew not unduly concerned about patient When the referral is for information only, or patient declines intervention by CCT. Who have non-reversible risk factors e.g. Dementia Virtual ward admin to send letter, a copy of Staying Steady and info on Steady & Strong classes (if appropriate) to patient and covering letter to GP (templates provided)
