Purpose of the team book Maintain records of the players Include parental consents and contact information Head coaches clinic certificates Include emergency contact information for players Presentation of team book Ensure that all forms are dated 2017 Forms should be placed in a three ring binder Use sheet protectors to separate information Team books must be present at every: Team, Association, League and Regional event including games, practices, fund raisers, competitions, etc Spot checks should be conducted by association and league officials Team books can be use in an emergency to access: 1. Final Skill Progression Sheet/Head Coach Declaration (Spirit Only) Participant Order in the Book ALL participants for football and cheer should be listed in alphabetical order. All Older/Lighter football participants should have a RED tab on their ID Card that say O/L for Older Lighter National Certified Team Rosters Certified & signed by League official  “Do Not Use” roster is not acceptable Coaches/Assistant coaches certification Football Staff ALL staff (including team parent & coach trainees) on the football roster must complete the USA Football Level 1 Coach Certification Course. New coaches take the full USA Football Level 1 Course. Coaches are still required to complete the SE Regional Training. Team Parents are also required to complete the SE Regional Team Parent Training. Spirit Staff ALL staff (including coach trainees and student Demonstrators) on the cheer roster must complete the Youth Cheer & Dance Alliance (YCADA). Coaches are also required to complete the Southeast Region Coaches Clinic. Team parents MUST have YCADA training. Coach Certification Expirations USA – 1 year PSC – 1 Year YCADA – 2 years SER Cheer Online Temporary Basic – 1 Year SER Cheer Advance – 3 years, 2 years starting 2018 SER Football Advance – 3 years SER Football Online Temporary Basic – 1 year SER Team Parent Live or Online – Forever (subject to change) SER Tiny Mite Football – Forever (subject to change) Everyone on Football roster needs this certification. First Aid and CPR Certification Include a copy of each card holders certification Student Demonstrators and Coach Trainees have the same paperwork requirement as all other participants. Coach Trainees & Student Demonstrators Should be listed as participants, and also should have certificates listed in the back of the book.

Certified National Roster Place a certified roster into the book. This is the only thing in this sleeve. If the roster prints two pages, then do not staple. Make sure the second page can be read from the back of the sleeve. Sleeve #2 League One Roster This is printed out from League One.

4 Sleeve #3a This is required to be filled out for every team book
Emergency Action Plan Insert your Association’s Emergency Action Plan. A great action plan gives instructions on what to do in the event of an emergency, the 911 address for the field of play, and a list of important phone numbers of people to contact on sight should an emergency occur. Sleeve #3a This is required to be filled out for every team book

5 Association Designated Player Safety Coach
Please fill out this attestation form for each participant that completes the PSC training. Sleeve #3b Each park has one trained and certified USA Football Player Safety Coach.  The Player Safety Coach's USA Heads Up Certificate is placed here.

6 CPR Certified Identification Cards
Insert the certificate of the CPR & First Aid Sleeve #4 There must be a minimum of one adult on each team roster that is CPR certified.  Copies of the CPR certification cards from the certifying company are placed here.

7 Sleeve #5a Front -Official ID card Verify form date
Complete by Association New Box Certification Section Sleeve 1 front, Participant ID card Print this card on 80lb cardstock in the color that matches the division as listed at the top of ID Card (see card color by division section printed on the cards). Participant Photos Photos should fit inside the designated box. All photos should be tightly framed head shots taken from the shoulders up. Make sure photo is taken in focus, with no hats, helmets, or sunglasses. Please no posing. The picture must be firmly secured to the card. Print photos in full color on matte paper, no high gloss paper, no black & whites photos accepted. League stamp should be partially on the card and partially on the picture without covering the child’s face. Once the photo is stamped, the photo may be taped down with clear packing tape. Initial weight (Association/Parent Section) Enter weight on “initial weight” line; This weight must be done in conjunction with the first practice; and must be within mandatory cut limits (no more than 6.4lbs over, 3lbs under). DOB Date of Birth on the Birth Certificate O/L It needs to be the O/L stamp in red ink (football only) Medical Y/N (circle one to verify examination of physical form) Medical Provider Type (write the credentials of the Licensed Examiner who signed the physical example: DR, DO, ARNP ) Certified Weight and Date Taken prior to the first regular season game for the League during the Certification days established by the League Certification weight can ONLY be done by League official Spirit Certified Date Date the cheerleader/dancer is certified Scholastics Insert actual GPA or circle SE (Scholastic Eligibility) as applicable. Must match what is printed on the scholastics report on the roster. It has to be signed and dated by the League Certification Official Game Date Certification Game Date Date of Game Weigh Master’s Certification League or Association Stamp ( Every game every participant) P - Pass – Certified to participate that day F – Fail – Failed to certify to participate that day DNP – Did Not Participate (Absent, Sick, Injured) Spirit Only – If a cheer participant is at the game but does not participate – stamp the ID Card in the appropriate box and circle DNP If a cheer participant is absent from the game do NOT stamp the box – simply write ABSENT in the Weight Master’s Certification box. DO NOT USE “WHITEOUT” ON THIS FORM! The front of this sleeve must be cut out on the bottom section to allow the card to be stamped without removing the sheet each week. The Association/parent fills out the top section with the participant’s information. The league will complete the second section at book certification. The League adds the ‘certified weight & date’ for football, for Spirit the League adds the ‘certified date’. Once the participant passes certification, the League Official signs or stamps the ID card.

All Legal paperwork including: Examples: Waivers-(1st) Name Changes Guardianship Adoption papers Name Changes; Guardianship paper work; Court orders; and Any other relevant documentation.

9 Foreign Language Birth Certificate Sample Translation Form If applicable should be in Sleeve 5b
If the birth certificate is in a foreign language, a notarized document translating the vital information into English should be attached.

10 Highlight Name and Date of Birth League Stamp in clear area Sleeve 1 back Birth Certificate The stamp verifies comparison with original document to the certified copy. Stamp in clear area. See arrow example on PowerPoint. This is verified by a League official. Make sure to double check name of the child & the date of birth, not the date filed fields, a common mistake. Highlight in YELLOW - Name and DATE OF BIRTH If you have a foreign language birth certificate - you must attach a notarized document translating the vital information into English. A certified copy of the birth certificate on file bearing the seal of the issuing office of the state of birth is the best guarantee of reliability of claimed date of birth and is form of proof most recommended. Passports are also reliable. Military ID cards are acceptable. Certified wallet-size certificates issued by a state or commonwealth are acceptable. Any other alleged “proof of birth date,” including photo copies of “originals,” are to be accepted only upon the willingness of the team administration to have its schedule forfeited should fraudulent application later be determined. PWLS does not mandate retaining the originals but verifying them. Sleeve #5b Back - Birth Certificate (an original certificate or a certified copy) The League Official should stamp in a clear area on the copy which verifies they have compared the original certified document to the copy. If the birth certificate is in a foreign language, a notarized document translating the vital information into English should be attached. (Sample translation form on next slide). Tip: When comparing the birth certificate to the ID card and the roster, make sure to check the spelling of the child's name and verify the date of birth was used instead of the date filed as this is a common mistake.

League Only Highlight Year of Form All this area needs to be filled out also League Stamp Here Page 2 front Physical Form Section II of Medical History (This section must be shown) Verify the current year’s form is being used. Highlight in yellow the year at the top of the form. Verify that the Physical was performed by an approved Licensed State Examiner. (See next slide for a list of approved providers in the State of Florida. Georgia will need to verify within their own state). Only the following can sign a physical in the State of Florida- MD’s, DO’s ,PA’s, ARNP’s, CNS’s, DC’s (Chiropractor). RN, Medical Assistants, LPN, CAN, Athletic Trainers, certified/ registered Chiropractor Assistants ARE NOT allowed to sign this form within the State of Florida, these physicals will be rejected. Required Medical follow-up must be accomplished as specified and/or prior to certification. If physical is presented on a form other than the Pop Warner form, then the required language must be on the other form. Please refer to the rule book for the current language or the current physical above the doctor’s signature. The parental page of the Pop Warner physical will still be required even if another form is provided. The other form will also need to have a Provider’s signature dated after January 1 of the current year. Sleeve #6a Approved License Medical Examiner’s Signature Page No Whiteout The League Official should verify the following: the participants name, the physical section is filled out, the Dr. has signed or stamped the form, the Date of Signature is in the current year, the medical profession is on the list of approved degrees (see attached list) and if the Dr. has indicated that follow up is required that there is documentation this has been done. **NOTE: the following professions are not permitted to perform physicals - RN, Medical Assistants (MA), LPN, CAN, Athletic Trainers, Certified or Registered Chiropractor Assistants, Acupuncturists.

The participants name needs to be filled out. Also the participant needs to sign. Sleeve #3 Back- Participant/Parental Consent Form Sleeve #7 a & b The League Official is verifying the following on this form: The person signing the form is the parent or legal guardian and the form is dated The participants name is on the form The participant has signed the form (regardless of how young)

16  Sleeve 4 Front Scholastic information
Participant must submit report card Final Report Card from previous school year; OR Tiny Mite form (SE Region form). Order- 1. GPA calculation sheet 2. course work sheet 3. report card Sleeve #8 Front- GPA Spreadsheet with full report card behind the spreadsheet The names on the GPA Spreadsheet and the report card must match. The report card must include all 4 – 9 week periods or semesters from the previous year. This sheet can ONLY BE STAMPED by the League Scholastic Coordinator. See above for correct format of stamp.

17 Absentee Form Absentee Form If a person is absent from an official Association, League, Regional or National Event they are required to have one of these forms on file in the book. The form should be filled out in advance for the event that they are planning to miss, and placed in front of the ID card, then after signed and stamped, moved to sleeve 9 for safe keeping. If the dates missed are consecutive, then they can all be placed on the same form. Example, a player becomes injured and missed 3 games in a row before returning. All three games may be placed on the same absentee form, then attach the doctor’s note explaining the injuring. When the player is cleared to return, obtain a doctor’s note and attach it to the absentee form or the physical. Sleeve #9 if a person is absent from an official Association, League, Regional or National Event they are required to have one of these forms on file in the book. The form should be filled out in advance for the event that they are planning to miss, and placed in front of the ID card, then after signed and stamped, moved to sleeve 9 for safe keeping.
