Open Enrollment Period For Plan Year 2018 ~ Friday, September 29th - Friday, October 27th at 11:59 P.M. Make Any Changes During This Period Add/Drop Eligible Dependents Elect Flex Spending Accounts (FSA) Enroll/Terminate Other Benefits NO Changes Permitted During the Year without a Qualifying Event

City Premium Payment Plan
Employees are automatically enrolled in the City’s IRS Section 125 Qualified Premium Payment Plan: When Electing Benefits: i.e. Medical, Dental, Vision premiums are deducted Pre-tax from your paycheck. No Changes to Elections Except for Qualifying Events (30 days to notify HR of Qualifying Event, per IRS Section 125)

Dental Insurance 2018 If you are currently enrolled in a Humana dental plan you will be required to chose another dental plan! Dental coverage will not roll over from 2017 If you do not enroll in a new dental plan for 2018, your 2017 dental coverage will be terminated on Dec 31st with NO OPTION to re-enroll after October 27, 2017 and no dental coverage for 2018.

Administered by Aflac through WageWorks. Flexible Spending Account limits~ $2,600 for Unreimbursed Medical $5,000 for Dependent Care If you did not keep your FSA debit card, you may request a new one online through the EBC FSA requires a new online enrollment each year, it does not roll over!

Enrolling in Your Benefits for 2018
You only need to go into Bentek if you are: Currently Enrolled in a Humana Dental Plan and would like to continue dental coverage. You must enroll in a different plan or lose dental coverage! Making changes to Benefit Elections for 2018 Adding/Removing Dependants to Your Plans Updating your Beneficiaries Enrolling in your 2018 Medical or Dependant Care Flexible Spending Accounts IMPORTANT! FSA’s do not automatically roll over – Must Enroll Annually!

If waiving medical insurance coverage, you must provide proof of other coverage for yourself only and complete a waiver form. You must meet with Aflac representative Frank D’Ascoli if you are adding, dropping, or making changes to your supplemental insurance elections. If you make changes in the Bentek system, you must click on “I Agree” and “submit” on the final page. If you exit before clicking “SUBMIT,” your changes will not be saved.
