Lifelong Services Context
Aspiration: Those with Lifelong Disabilities (including Sensory Impairment) acquired before the age of 25 and autism are as independent and resilient as possible and able to live a ‘good life’ based on what is important to them. Lifelong Services Transformation Programme April 2018 Workstreams Bringing Teams Together A unified Service with relevant teams and staff across social care , health and education to facilitate and support a consistent approach to children, young people and adults with disabilities or autism (lifelong or acquired before the age of 25) Developing the Model Together A person centred model of practice and streamlined pathway across LLS, which enables early intervention, facilitates independence and resilience, and improves experience for staff and citizens Thinking Differently Establish a shared culture and good practice across children's, adults’ and education to achieve inclusion, independence aspiration and resilience for children, young people and adults with disabilities and autism, and cost effectiveness for the Local Authority Budgets Together Effective whole life budget planning for Children, YP and Adults with disabilities and autism, supporting improved efficiencies across the LA Planning Together Appropriate support and provision for all levels of need across Children, YP and Adults with disabilities and autism focused on early intervention and prevention to achieve inclusion, independence aspiration and resilience, and cost effectiveness for the Local Authority Co-Production The voice and views of children, young people and adults with disabilities and autism are at the centre of the programme particularly the Planning , Developing the model and Thinking Differently workstreams
