A unified Service with relevant teams and staff across social care , health and education to facilitate and support a consistent approach to children, young people and adults with disabilities or autism (lifelong or acquired before the age of 25)
Developing the Model Together
A person centred model of practice and streamlined pathway across LLS, which enables early intervention, facilitates independence and resilience, and improves experience for staff and citizens
Thinking Differently
Establish a shared culture and good practice across children's, adults’ and education to achieve inclusion, independence aspiration and resilience for children, young people and adults with disabilities and autism, and cost effectiveness for the Local Authority
Budgets Together
Effective whole life budget planning for Children, YP and Adults with disabilities and autism, supporting improved efficiencies across the LA
Planning Together
Appropriate support and provision for all levels of need across Children, YP and Adults with disabilities and autism focused on early intervention and prevention to achieve inclusion, independence aspiration and resilience, and cost effectiveness for the Local Authority
