1 An Introduction to West Moreton Oxley Partners in Recovery

2 What is Partners in Recovery?
It is an innovative, national 3 year program funded by the Federal Department of Health The Program is based within the 61 Medicare Local regions The regional models vary according to local needs In the first rollout of the initiative, 49 Partners in Recovery organisations were funded in 2013 Aftercare is one of 4 non Medicare Local Lead agencies appointed to implement PIR

3 Five eligibility Criteria for West Moreton-Oxley Partners in Recovery
Appears to have a mental illness that is severe and persistent Have complex needs that require support from multiple agencies Requires substantial support and assistance to engage with the various services to meet their needs No existing care co-ordination efforts are in place, or those arrangements have not worked and are likely to be addressed by acceptance into PIR Be willing to work with us or their legal guardian has indicated willingness to participate in PIR

4 How can we help? We recognise that people with severe and persistent mental illness have more complex needs than current systems can meet PIR aims to partner with key services and supports from multiple sectors to work in a more collaborative, coordinated and integrated way to improve system responses

5 We are unable to: Provide formal mental health assessments (although we can organise assessments when they are needed) Provide ongoing case management and support services (we help to coordinate services rather than directly provide services ourselves) Provide a crisis intervention response or after-hour service

7 West Moreton-Oxley Partners in Recovery aims to:
Work in partnership to make access easier and stronger for people with severe and persistent mental illness who have complex needs that are not being met Understand system issues and work collaboratively to improve system responses for people with severe and persistent mental illness Facilitate sustainable change which will make client access to services easier and stronger in the future.

The blue team are responsible for practice management/supervision. The contractors are responsible for operational line management and SFs direct work with clients. Bottom up and top Down approach Working groups being influenced by themes and issues on the ground and the common agenda of the consortium The success of working groups will depend on getting the right stakeholders and partners on board for active participation to initiate change.

Our task is to work with the client group and collect the evidence of gaps and barriers and feed the issue up to the system coordination level. The Systems coordinators, CMC and PIR Management team work at a strategic and regional level to lobby for systems change. 16 Flexible funding The Partners in Recovery program has made available flexible funds. These have two targets: Individual Partners in Recovery consumers or groups of consumers; and Regional reform

17 Individual Flexible funds
Each host agency has an individual flexible fund budget Flexible funds are not emergency relief and not crisis funds They are to be well utilised to leverage change for a client to close service gaps and to reduce barriers to recovery
