Overview Virginia Department of Health Updates Topics include: ConnectVirginia All Payer Claims Database ED Care Coordination Project Vital Records Data Sharing / Governance / Analytics Security 2

3 ConnectVirginia Connect Virginia coordinates and hosts the Virginia Veterans Lifetime Electronic Record regional meetings Collaboration with CRISP to expand Encounter Alerts to Virginia residents in Maryland and DC facilities Enhanced advance directives registry and bi-directional state immunization registry Management Services through Virginia Health Information Offerings now include the following: Exchange, public health reporting portal, advance health care directives registry, basic encounter alerts ConnectVirginia is also working with VDH and other stakeholders to implement the Emergency Department Care Coordination program

4 Virginia’s All Payer Claims Database (APCD) 2017 Accomplishments
In 2017, the number of registered users rose to over 260 Added Medicare Fee-for-Service data to the APCD, which includes paid claims for over 1 million Virginians. In order to do this, VHI had to become certified as a CMS Qualified Entity. Initial certification was awarded in June 2016 and VHI obtained 2015 and 2016 Medicare Fee-for-Service data. VHI also applied to obtain authorization to use the data for VDH to use for public health initiatives. This is awaiting final approval from CMS. APCD In Action Population health infographics and whitepapers Virginia Center for Health Innovation’s Health Value Dashboard initiative and similar dashboards for FQHCs Choosing Wisely and the MedInsight Health Waste Calculator Healthcare Pricing Report to increase transparency to consumers Working to obtain paid claims data from the Federal Employee Health Program and TRICARE, and expand to more self-insured employers This are the major topics that we are going to cover. I have prepared about 40 slides, but several I will spend more time on some than others.

5 Emergency Department Care Coordination Program
Program Overview: Provide a statewide technology solution that connects all hospital emergency departments to facilitate real-time communication and collaboration amongst health care providers Enable the exchange of real-time patient visit information and integration with hospitals’ electronic health records systems Ensure executed data exchange contracts exist with participants to ensure privacy and security of patient information

Enable receipt of real-time alerts to identify patient-specific risks, to create and share care coordination plans, and to assess other clinically beneficial information Provide the patient’s primary care physician and managed care organization with treatment and care coordination information Integrate with the Prescription Monitoring Program and the Advance Health Care Directive Registry Funding for the project – General Funds and HITECH

Accomplishments: Vendor has been secured: Collective Medical Established the ED Advisory Council (ED Council) “Road Shows” with Hospitals/Health Systems and Health Plans Two main workstreams: technology and data sharing/trust agreements Eleven hospital systems are in process on the technology piece

Next Steps: Create detailed implementation plans with each system and health plan as we complete these “road shows” Implement the solution in hospital EDs across the state and with Medicaid Managed Care Plans by Jun 30, 2018 Integrate the solution to the Prescription Monitoring Program and the Advance Directive Registry Expand the program to downstream providers such as FQHCs and CSBs Develop metrics to analyze the success of the program Create the Clinical Consensus Group

9 Vital Records Systems Updates on Electronic Death Registration System:
System is currently voluntary; several bills in this year’s General Assembly seek to make this mandatory Almost all funeral homes are participating and around 1200 physicians Bringing on UVA and VCU before April 2018 Data Sharing – death information, etc.

10 Data Governance Vision: A sustainable, collaborative environment promoting data excellence throughout the Virginia Department of Health. Highlights: Standardized, flexible methodology for sharing data Secure data transport available for use at no cost Executive, Management, and Staff level engagement Framework for institutionalizing dialogue and feedback Improving coordination between different elements of VDH

11 What’s on the Horizon? Electronic Health Records system for VDH?
Implement ED Care Coordination in all emergency departments by June 30, 2018 Continue to build on this program by convening the Clinical Consensus Group to help determine next steps Interoperability Data sharing – open data
