1 Purpose The Health Care Payment Learning & Action Network (LAN) was launched because of the need for: The Health Care Payment Learning & Action Network (LAN) was launched because of the need for better care, smarter spending, and healthier people. The LAN is about shifting away from the current fee-for-service model of payment in our health care system (where more services leads to higher payments, regardless of health outcome) to one that pays providers and hospitals for quality care and improved health. In order to achieve this, payment structures need to be shifted to quality of care over quantity of services. The LAN is a collaborative network of public and private stakeholders, working together through various groups in the health care community to create awareness. Better Care The LAN seeks to shift our health care system from the current fee-for-service payment model to a model that pays providers and hospitals for quality care and improved health. Smarter Spending In order to achieve this, we need to shift our payment structure to pay for quality of care over quantity of services. Healthier People Such alignment requires the participation of the entire health care community. The LAN is a collaborative network of public and private stakeholders.

The LAN’s mission is to help achieve better care, smarter spending, and healthier people. The Department of Health and Human Services (HHS) is working in concert with partners in the private, public, and non-profit sectors to transform the nation’s health system to emphasize value over volume. Mission Better Care, Smarter Spending, Healthier People To accelerate the health care system’s transition to alternative payment models by combining the innovation, power, and reach of the private and public sectors.

3 Whether you are a patient, a provider, a business, a health plan, or a taxpayer, it is in our common interest to build a health care system that delivers better care, spends health care dollars more wisely and results in healthier people. Today’s announcement [of the launch of the LAN on March 25, 2015] is about improving the quality of care we receive when we are sick, while at the same time spending our health care dollars more wisely. Sylvia M. Burwell, Secretary- U.S. Department of Health and Human Services (HHS) HHS launched the LAN (on March 25, 2015) to help advance the work being done across sectors to increase the adoption of quality-based payments and alternative payment models. Opening remarks were made by Sylvia M. Burwell (HHS Secretary) and President Obama to participants across the health care community, who are valuable stakeholders in increasing the awareness and the adoption of the alternative payment models (APM).

4 Our Goal 2016 30% 2018 50% Goals for U.S. Health Care
Adoption of Alternative Payment Models (APMs) 2016 30% In 2016, at least 30% of U.S. health care payments are linked to quality and value through APMs. HHS set a goal of tying at least 30 percent of Medicare fee-for-service payments to quality or value through alternative payment models by 2016 and at least 50 percent by The LAN extends those Medicare goals to the private sector and states. As a public-private effort involving diverse stakeholders across health care, to date over 6,000 individuals are participating in the LAN. 2018 50% In 2018, at least 50% of U.S. health care payments are so linked. 5 Develop Recommendations
Operational Model Critical path to broad adoption of Alternative Payment Models (APMs) Through its activities, the LAN is about “force amplifying” successful innovations in the field in order to accelerate change. The LAN strategy involves convening the nation’s leading public and private APM experts and experienced practitioners to: Share promising practices, through webinars, interviews and blogs, and in-person Summits Establish a framework to categorize APMs and use the framework as a roadmap Develop and then road-test recommendations for increasing alignment And track the nation’s progress toward the 30 and 50% goals Establish Framework Gather Innovations Develop Recommendations Drive Alignment Demonstrate Results APM Framework Guiding Principles Leadership Groups Partnerships Research LAN Engagement Population-Based Payment Models Clinical Episode Payment Models Implementation Resources Learning & Sharing Measure & Track Progress Pilot Recommendations Action Collaboratives

6 Leadership Groups Providing leadership and coordination of LAN activities and priorities All payers, health care providers, employers, states, consumer groups, patients, and others are invited to participate. Participants may apply to serve on Work Groups or join Affiliated Communities, or may get involved in Affinity Groups to engage with the LAN and help increase adoption of alternative payment models and other care delivery models. Most meetings of the LAN occur virtually by teleconference or webinar. Guiding Committee Primary leadership body of the LAN. The GC meets monthly, establishes and oversees work groups, and actively engages stakeholders across the LAN. Work Groups Short-term, multi- stakeholder initiatives of experts charged with identifying and assessing the primary barriers to adoption and outlining key steps toward the achievement of goals. Affinity Groups Venues for participants in specific sectors, such as employers/purchasers, to engage around specific topics and to identify and disseminate knowledge and best practices.

Leadership Groups LAN has established seven groups with varying purposes. Guiding Commit The LAN began in March 2015 with a kickoff event at the White House with 230 participants attending. Based on feedback generated at that event, the LAN Guiding Committee (GC) was established and had its kickoff meeting by teleconference on May 27 during which it laid out initial priorities. The GC has established Work Groups, Affinity Groups and Action Collaboratives to carry out aspects of the LAN strategy.
