Full General Medical Services are available at the practice including: General Health Checks Family Planning Women’s Health Children’s Health Limited Travel Health and Vaccinations Pre and Post Natal Services Minor Surgery Wound Care Chronic Disease Clinics Diabetes and Asthma Clinics INR Checks Phlebotomy (Blood Tests) Private Fees: Not all services are provided free by the NHS. Private medicals, copies of records and some vaccinations are subject to fee. Reception team will provide further details. Staff at the surgery Practice Nurses: Nurse appointments are made through reception. Reception: Team of 5 staff who will help you book and appointment and manage any queries you may have. Admin Team: Practice Manager, Assistant Manager, 1 Admin and 1 Secretary. Health Visitors (HV): The Ealing and Hanwell HV team run a Duty Desk and can be contacted on Mon – Fri. District Nurse Team: We are supported by a team of District Nurses based at Mattock Lane HC on All newly registered patients will require a new patient health check with a Nurse. Surgery Hours Monday – Friday 8am – 6.30pm Saturday Clinic pre booked appointments only The Care Quality Commission regulate and monitor the level of care and services we provide. They can be contacted on Overnight, at weekends and bank holidays, NHS 11 1 provides out of hours services. Calls to 111 are free from mobiles and landlines.

2 Patients Rights and Responsibilities
Appointments All patients are seen by appointment. Please arrive in good time or cancel if no longer required. We offer the following kinds of face to face appointments. Call on and listen to the options. Alternatively you can book your appointments on line, ask Reception for details. Routine: Bookable via reception and online, up to 4 weeks in advance. You can book an appointment with a Doctor or a Nurse. These 15 minute appointments are available for all routine and non-urgent problems. 48 Hour: Also bookable via Reception and online, release 48 hours in advance. These 10 minute appointments are more limited in number and should be used for things that are urgent but do not need to be seen on the day. Same Day: Released only by the Duty Doctor following a telephone triage consultation. These 7.5 minute appointments are released by the Duty Doctor if required. It is important that you call the surgery from 8am and before 1pm if you have an urgent medical condition or query. Home Visits: We visit patients at home where medically necessary. Your request will be taken by then Duty Doctor, please phone the surgery before 11am. Results: Call Reception for test results between 12pm – 6.30pm Bloods: If a Doctor or Nurse require you to have a blood test, phone Reception who will book you in with our Phlebotomist who is at the Practice Wednesday’s 8.30am – 12.30pm and Friday’s 8.30am – 12.30pm. Repeat Prescriptions Repeat prescription must be requested in writing or online. Please allow 2 working days for this service. If you want to collect your prescription from a Pharmacy please make this arrangement with them personally. Patients Rights and Responsibilities Treatment/referrals are subject to patients giving informed consent to the clinician. We offer health checks for all patients registered with the Practice. If you more or change your telephone number please let us know as we may need to contact you urgent. All patient have an allocated GP, however you may choose the Doctor you wish to see if they are available. You can make a significant contribution to your own, and your family good health and wellbeing by following the courses of treatment you have agreed to, keeping your GP and hospital appointments, or if you have to cancel, doing so in good time. Aggressive/Abusive Behaviour The practice operates a ‘zero tolerance’ policy to aggressive and abusive behaviour. Any incident of verbal abuse whether in person or over the phone, is reported to the Practice Manager. Incidents are investigated and may result in the patient’s removal from the Practice list. Registering with Elthorne Park Surgery To register speak to Reception who will supply you with the necessary documentation to complete. We also require proof or identity and dressed. We accept the following: Proof of Identity: Passport, Drivers Licence, National Health Card Proof of Address: Utility bill, Council Tax bill To register with online services you also require the above documentation and must be over 16 years of age. Ask Reception for further details. Disabled Access: All clinical rooms are on the ground floor and toilet facilities for the disabled are available. Data Protection: We adhere strictly to the NHS guidelines and GDPR regarding personal information held at the Practice. Teaching: We are affiliated to The Imperial College School of Medicine and are involved with the teaching of student Doctors and Nurses. If you do not wish to have a student present in your consultation inform Reception. Compliments, comments or complaints: The Practice appreciated all forms of feedback. Please use the suggestion box in the waiting room. Most issues can be resolved without you having to make a formal complaint. Try having an informal chat with your Doctor or a member of staff first. A formal complaint takes time and mire issues are resolved quicker if you just speak to a person on site.
