5 Medication Release Document
Where the medication is being transferred to and from Medication name and strength Total amount of medication transferred Dated signature of person transferring and receiving medication The Medication Release Document is the Transfer Form. It is one of the tracking documents in the Chain of Custody. See Pg and Pg. 175 RIA Curriculum.

6 Transfer Form Review components of your agency transfer form. If unable to present, review this sample form (also found on Pg. 176 RIA Curriculum).

7 Medication Transfer Medication must be transported by MAP Certified or Licensed Staff Not allowed to send with individual or have non certified/licensed staff transport medication. Also, transporting of medications by MAP Certified staff must be done during the staff’s normal working hours and the transported medications must be for the individuals residing at the MAP site where staff formally work. MAP staff must not transport medications for individuals residing outside the staff’s work site or on the staff’s own time.

8 Day Program Medication
Ask pharmacy to ‘split package” medication Whenever possible medication should be scheduled to be administered in the residential program. However, if a person will routinely be receiving medications at more than one location, such as the day program, ask the pharmacist to ‘split package’ the medication. A separate tamper resistant package will be prepared by the pharmacy that contains only the amount required for day program administration.

9 Day Program Medication
Before Sending Document in Medication Ordering and Receiving Log as received in residence/add countable medication to count Complete a transfer form If countable medication, subtract from residential program’s count book

10 Day Program Medication
At the Day Program Obtain dated signature of person accepting medications If countable medication, day program staff must add to count book Make a copy of transfer form and give to day program Return transfer form to residential program Only MAP Certified/Licensed staff may accept the medications at the Day Program and sign transfer form.

11 Day Program Communication
Residential staff’s responsibility to ensure the day program has all required information for medication administration Including current copies of individual’s HCP orders Communication is essential for safe medication administration Review class discussion Pg. 178 RIA Curriculum Review the communication system between the day program and your residence.

12 Central Location Disposal
If medications need to be transferred for disposal to a Central Location Before Sending Complete a transfer form If countable medication, subtract from residential program’s count book Transport to Central Location Central Location must be a MAP Registered site(current MCSR). Transport must be done by MAP Certified /Licensed staff. Transporting of medications by MAP Certified staff must be done during the staff’s normal working hours and the transported medications must be for the individuals residing at the MAP site where staff formally work. MAP staff must not transport medications for individuals residing outside the staff’s work site or on the staff’s own time.

13 Central Location Disposal
At the Central Location Obtain dated signature of person accepting medications Make a copy of transfer form and give to Central Location Return transfer form to residential program Only MAP Certified/Licensed staff can accept medications for disposal at a Central Location.

14 Central Location Delivery
If medications are delivered to a Central Location and then need to be transferred to individual residence At the Central Location If countable medication, must be added to Central Location’s count book and subtracted when transferred to individual residence Transfer form must be completed including dated signatures of person releasing and accepting medications Obtain copy of transfer form for residence Transport to residence Central Location must be a MAP Registered site(current MCSR). Transport must be done by MAP Certified /Licensed staff. Transporting of medications by MAP Certified staff must be done during the staff’s normal working hours and the transported medications must be for the individuals residing at the MAP site where staff formally work. MAP staff must not transport medications for individuals residing outside the staff’s work site or on the staff’s own time.

15 Central Location Delivery
Upon return to the residence Store medications securely If countable medication, add to residential program’s count book File transfer form at residence
