21 MRI interpretation There is hyperintensity on STIR sequences associated with the second metacarpal bone along its palmar, abaxial aspect, as well as the palmar aspects of C2 and C3 (medially). Heterogenous signal intensity is seen palmar to C2 and C3 on all sequences, and this abnormal tissue separates the inferior check ligament from the flexor carpi radialis distally. The proximal aspect of MC2 is markedly sclerotic, seen as hypointensity on PD sequences within the medullary cavity. No marked alterations in signal intensity are noted in the inferior check ligament or suspensory ligament. NOTE: more distally, the inferior check ligament became markedly enlarged (the patient had a history of previous inferior check ligament desmotomy).
