2 Definitions Servicing and Maintenance
Affected Authorized Servicing and Maintenance Servicing and maintenance includes activities such as constructing, installing, modifying, dismantling, setting up, changing tools, adjusting, inspecting, lubricating, cleaning, unjamming, and troubleshooting. Maintenance and servicing tasks may be performed by either maintenance or production personnel. Verification The authorized person applying lockout has the duty to verify that the isolation and de-energizing have been effective by attempting to start the equipment and release residual energy before starting servicing or maintenance work. Each authorized person installing a lock on an energy isolation device or group lockbox shall verify individually that the hazardous energy has been isolated and dissipated. Zero Energy State

3 Responsibilities Authorized Persons and Lockout Supervisors
Affected Authorized Authorized Persons and Lockout Supervisors Authorized persons and lockout supervisors shall be identified by site/operation; shall be trained and competent to fully understand the applicable requirements of this program; and be responsible for performing hazardous energy control in compliance with this program and applicable equipment-specific lockout procedures. Affected Persons Affected persons consist of all personnel who are not authorized individuals and who must receive lockout awareness training, which will include the importance of not attempting to start a locked out machine, equipment, or process. Affected individuals need to recognize and understand the lockout process.

4 LOTO(V) is Required When….
Affected Authorized One enters the Point of Operation; any point in the equipment movement or potential movement in which an employee could be injured Servicing and/or maintenance or any other time when an employee could or would become exposed to point of operation Servicing and maintenance may include tasks such as: Machine set-up Adjustments Inspections Cleaning Machine maintenance Lubrication Jam removal Tool changes

Affected Authorized Examples of production-related tasks requiring lockout: Cylindrical Grinder Change abrasive wheel Change centers Replace diamonds Change belts Change coolant filter Scrape out coolant sump Remove jammed/dropped parts CNC Machining Equipment Change collet/chuck Change machining fixtures Change cutting inserts Remove broken tools Pull chip conveyor Machine cleaning Remove jammed parts Entry into robot cell Entry onto AGV track area Single Source Crimpers Die Changes Lubrication Bar/Screw Machine Change tools, inserts or tool holders Change pusher tubes Remove jams from chip conveyor Entry into safeguards around bar pusher Broaches Change broach bar Change/adjust pullers Change fixtures Saws Blade Change Electrical circuit maintenance Cleaning of trays Coolant change

6 Affected Authorized Contractors

7 Contractors Affected Authorized During the contractor orientation Eaton will issue each contractor employee a sufficient number of RED locks The project coordinator will supply contractor management with a copy of the applicable equipment-specific lockout procedure Equipment will be locked out by an Eaton supervisor using RED locks and a lockout hasp in accordance with the applicable equipment-specific lockout procedure. Contractor personnel will then be required to install a RED lock on the appropriate energy isolation device prior to starting a job. RED locks must be returned to Eaton at the end of the project
