8:43e-14.3 Required Training a) Any employee hired before 9/18/2017 must be trained by 3/18/2018 b) Any employee hired after 9/18/2017, must be trained within 6 months of first day of employment

The online or in-person training entitled, Stop. Observe. Ask. Respond to Human Trafficking (SOAR): A Training for Health Care and Social Service Providers, published August 2016, by the United States Department of Health and Human Services, which incorporated herein by reference, as amended and supplemented. Registration for the SOAR training is available through the website of the Office on trafficking in Persons of the Administration for Children and families of the United States Department of Health and Human Services, at

Completion certificates are not issued

Facility has to ensure staff completed the training through facility practice, policy, and training records. One and done! This training is done only one time.

9 Health Care Professional Responsibility and Reporting Enhancement Act
The CN-9 form is to be used by an inquiring health care facility, licensed by NJDOH, to make an inquiry to another health care facility, licensed by the NJDOH, about a health care professional who is currently or was formerly employed at the facility. The licensed health care facility must complete the CN-9 It can not be done by a third party.

To find the CN-9 form and instructions: Go to NJ.gov then click on Department and Agencies next click on Health next click on Health Systems next click on Healthcare Facilities and Services next click on Facility Licensing and Inspections next click on License Forms finally click on CN-9

If CN-9 form is received, it must be completed and returned within 8 business days If CN-9 form is not returned, you can be fined.
