2. The TEC Journey so far… At the start in 2016: Now in 2018:
Huge Potential of TEC in tackling challenges in Health and Social Care
low baseline – traditional Telecare and community alarms such as Careline citizens connected
Appointed TEC lead and developed a TEC Strategy
A Bolton-wide Strategic Group driving TEC (Council, NHS, Housing, VCS);
Successful application for GM Transformation Fund (March 2017);
Now in 2018:
Integrated Telecare AND telehealth programme – one of the few in UK.
Contracted with Good quality TEC providers - proven and evidence based track record in delivery.
Now have a variety of TEC solutions to assist in meeting a range of needs across Bolton.
TEC is now in place for an estimated 10,300 citizens

3. TEC Solutions in Place
Telehealth (Care Homes): Care Homes connected by video link to clinical hub to reduce ambulance journeys and A&E admissions;
Telemedicine: up to 200 patients with long term conditions to be monitored remotely using TEC;
A year on year increase in telecare solutions for adults living at home;
Telecare assessors: 2 new staff in post to promote and increase assessments for and installations of telecare;
Telecare Practice Guidance developed and made available to all social care staff to promote “telecare first”;
TEC in LD Supported Living: TEC introduced to increase independence for adults in Supported Living;
TEC Training: all social care and health staff in Bolton to get access to basic training through digital channels;
Orcha: clinicians to prescribe health apps to specific cohorts (incl. Diabetes);
Brain in Hand: young people and adults with autism and Mental Health conditions to use TEC to become more independent);
AskSara: a web-based tool for citizens and professionals to identify the right TEC and equipment for them to remain independence

4. Some Expected Outcomes
TEC is embedded in all care delivery to deliver key outcomes:
Getting the right personalised support in using TEC solutions (early intervention, re-ablement, Discharge to Assess);
Increase in quality of life - being independent and self care using e.g. Brain in Hand to help people manage day-to-day life;
More self care by making tools available (e.g. AskSara, Orcha mHealth);
Decrease in pressure on social care system (self care, early intervention);
Decrease in pressure on ambulance journeys (self and remote monitoring of conditions);
Decrease in A & E admissions and increase discharges
Increased “buy in” from workforce – paid and unpaid carers in the system;
