1. Hospital Discharge Project
This service is designed to enable the Hospital to send you confidential information about a patient they have recently discharged. This could include changes to medication, new Dossette box information or a follow up request.
The other advantage is that by knowing your patient has been discharged, you can carry out either a discharge MUR (within a month) or potentially an NMS with them.
More importantly, you are able to ensure they are happy with their medicines and understand how and when to take them as the ideal outcome is that they are at a reduced risk of readmission.

When you log on to PharmOutcomes you need to go to the Service Tab. Any discharge/Dosette box information will be listed above your messages.
If they are one of your patients you will need to accept them and then ensure you press the Complete button. This will ensure that the system registers the fact that you are dealing with the information. If they are not one of your patients you will need to reject them with a reason
When you go into the service on PharmOutcomes there will be either 1 or 2 pieces of information from the hospital one of which may be a TTA (To Take Away) medicines list or a discharge letter which you can use to ensure your records are correct. .

3. You wont see the service listed on your normal menu
You wont see the service listed on your normal menu. New Referrals will be listed at the top of your services list as “Outstanding referrals”
