2. In this workshop we will discuss: Why do we have THP-Plus Provider Plans What needs to be in a Provider Plan, and How can Provider Plans be used to improve practice

3. Why…… Do We HaveTHP-Plus Provider Plans?
CDSS requires all THP-Plus Providers have a THP-Plus Plan on file.
Counties are responsible for certifying that Provider Plans meet all the requirements set forth in the regulations.
Provider Plans are the foundation for the THP-Plus County’s THP-Plus Program.
Provider Plans feed into the overall County THP-Plus Plan.

4. Logistics of the Provider Plan
Provider Plans need to be certified by the County prior to THP-Plus services being provided.
Provider Plans need to include all required information.
Provider Plans should be reviewed and updated annually.
Provider Plans must be kept on file at the County Social Services Office.

Promote accountability:
Check and balance for the program
County and the Provider
Manage the THP-Plus Program:
Program Monitoring – the program being offered should reflect the program being provided
Fiscal Monitoring – what happens when the program expenses are reconciled at the end of the year and there are remaining funds?
Staffing Accountability – Staffing Work Plan
Program Improvement
Consisting amongst programs in a County

7. When Should Provider Plans Be Updated………
Provider Plans should be updated when changes occur in the program, such as
Staffing
Budget Modifications
Program Elements (Adding or Subtracting)
Outcomes
The County THP-Plus Plan has changed
