1. How to Schedule an Appointment at an Onsite Event or Patient Service Center

If you are a new user, you will have to register on this new site as a new user.
If you are a first time user, click Enter Registration Key, then Continue 1
*Important: If you have already have a username and password from last year, you will not set up a new account. You will click sign in now.
First time users, enter the Registration Key (Alief) and click “Continue.”
Enter your Unique ID (this is your 6 digit employee ID ), and Date of Birth.
Create an account by entering a username and password,  to accept,
then click “Continue” 2

2. 4 5 6 Click “Get Started” to schedule your appointment.
Select how you would like to participate in a screening : At an onsite event or a Patient Service Center (PSC)

After clicking Get Started, a new box will pop up with “Your Screening – schedule now. Click schedule now to get to the next screen. 5
When you select “At an Event” or “At a Patient Service Center” you’ll need to choose the appointment location and then select a date and time. For PSC appointments, you may choose the walk-in option to create an order that is ready to be filled at any PSC nationwide..
Click “Confirm” to confirm your appointment. 6
Once you’ve scheduled your screening, you will see the Order Confirmation page.

3. How to use the Physician Results Form
Select Physician Results Form from your screening options to go directly to the Order Confirmation page.
Can you reformat the screen shots on set 1 and 2 so that the registration box is part of step 1 for all scenerios
Download and print your custom Physician Result Form from this page.
Have your physician complete the form, then send or fax the form to the party identified on the form.
