4 What is SOAR? A model for assisting eligible individuals to apply for Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI) For individuals who are experiencing or at-risk of homelessness and have a serious mental illness, co-occurring substance use disorder, or other physical disabilities Sponsored by the Substance Abuse and Mental Health Services Administration (SAMHSA) in collaboration with the Social Security Administration (SSA) since 2005 All 50 states and Washington, D.C. currently participate

Demonstrate a commitment to developing a stakeholder meeting and monitoring a SOAR Implementation Plan Identify staff to complete SOAR Online Course and applications Describe strategies for tracking outcomes Designate a Criminal Justice Lead who will convene the stakeholder meetings, conduct SOAR Fundamentals trainings, mentor trainees and track outcomes

20 Important Dates Intent to apply by November 17, 2017 (requested, not required) Applications are due January 8, 2018. Notifications will occur by January 17, 2018. The SOAR CJ lead will attend a Leadership Academy on April , 2018 in Boston, MA. The one-day planning implementation meeting must be held prior to June 30, 2018
