Medical Plans from PPO Plan 10B Plan Highlights:
Deductible is $4,000 single/$8,000 family – applies to ALL services EXCEPT preventive services Maximum out of pocket is $6,550 single/$13,100 family Must satisfy deductible and then copays would apply. Can utilize any provider or facility in local Personal Choice network and the National PPO Blue Card Network NO Referrals are required PREVENTIVE SERVICES = $0 COPAY! Can establish a Health Savings Account to pay for qualified medical expenses

Deductible is $2,000 single/$4,000 family. Deductible applies to some services, but not all. Delta Dental PPO Premier
Can use any dentist but greater savings using dentists in the PPO or PPO Premier Network Exams, Cleanings, Xrays, Sealants covered at 100% Deductible for other services: $50 single/$150 family Most services covered after the deductible at 80% or 50%. Dental Implants covered at 50% to calendar year maximum Orthodontia covered for children up to age 19: 50% to $1,000 lifetime maximum Calendar Year maximum: $2,000/PPO Dentist or $1,500/PPO Premier Dentist

Open enrollment deadline: Monday, February 20, 2017 for Medical and Dental Plan enrollment and changes. Make sure your beneficiary is up to date for your Solid Wood life insurance coverage!
