1 Employee Evaluations Employee evaluations are conducted at 6 months of service for new employees and annual on employee anniversary date. Leads receive the evaluations forms as a outlook task from HR Department. Leads will also complete Employee Accountability Checklist with the employee evaluation process Leads may review prior employee evaluations, and accountability checklists in Time Force under the HR tab for each employee that are in there department. Leads will need to run an attendance report from Time Force to review missed hours from work for the evaluation.

2 Employee Evaluations Top portion of form will be completed by HR Department. With employee name, title, hire date, years of service and current wage.

3 Employee Evaluation Lead will need to select the development level D1, D2, D3 or D4 for each category. The information was discussed in the One Minute Manager training If training needs are applicable, please complete. HR will help on the additional needs.

4 Employee Evaluations When the Lead has completed the employee evaluation, the lead needs to review the evaluation with the Plant Manager or Shop Foreman within the month the review is due. The discussion needs to be focused on the employee evaluation form that was completed and if a wage increase will be applicable. When the discussion has been completed with the Plant Manager or Shop foreman, it is time to schedule the review with the employee, please allow enough time for a good discussion.

5 Employee Evaluation Lead will need to complete new wage, if applicable and effective date, (must be a Monday) Please list employee comments received during review When evaluation is completed, the form will need to be signed by Employee and Lead

6 Employee Evaluation When employee evaluation and accountability checklist has been completed, scan and completed forms, and any other form that was discussed during evaluation to HR Department. HR Department will place completed evaluation and accountability checklist into time force and will complete a wage form (if applicable) and send to payroll department.
