1 Frontotemporal dementia – A retrospective study from a tertiary center .

2 Introduction Dementia is a silent epidemic , affecting the elderly with a staggering economical as well as emotional impact Frontotemporal dementia (FTD) is the clinical diagnostic term to describe patients with a range of progressive dementia syndromes associated with focal atrophy of the orbitomesial frontal and anterior temporal lobes . Epidemiological studies suggest that FTD is the second most common cause of young-onset dementia after Alzheimer’s disease

3 Methodology Study design : Retrospective chart review
All the case records of patients diagnosed as frontotemporal dementia , from 2002 to 2011 were collected from the medical records department , NIMHANS , through a computer run search for the ICD 10 code F02.0 All the files were reviewed for sociodemographic data , age at onset ,family history , EASI , HMSE scores , BPSD , comorbidities , imaging , treatment given and analyzed using SPSS 16

4 Results There were approximately equal number of males and females in the study population Mean age at onset was 54 years ,while mean age at first contact with professional was a year later Current study shows that the behavioural variant is 47% , semantic variant is 39% and progressive non fluent aphasia variant to be 14% Most common behavioural problem was aggression/agitation (41-43%) followed by disinhibition (40%) , repetitive behaviour (38%)

5 Most common psychological problem was low mood 40%
Most common comorbidity was diabetes(23.5%) followed by hypertension(21.5%) 10% of the study population had comorbid Extrapyramidal disorder . Mean HMSE score was 11.5 Mean EASI score was 10

6 There was no significant difference of presentation between urban and rural area

7 Around 48% of study population was educated upto 10th , while 28% were degree holders

8 Family history of dementia was positive in 12.5%

9 61% of the sample showed frontotemporal atrophy on imaging

10 The follow up rates were very poor and only 20% of the patients had come for follow up , the average duration of follow up was 7.5 months

11 Most common cognitive enhancer was donepezil (49%) followed by memantine(15%)

12 Most common antipsychotic used was quetiapine (60%)

13 Most common antidepressant used was fluoxetine (23%) followed by escitalopram (12.5%)

14 Discussion In the earlier studies there was male preponderance , however later studies didn’t prove this conclusively and even in the current study there was no significant gender difference noted Previous studies have indicated the age at onset to be 5th to 7th decade and the results of the current study are compairable The family history of dementia in the current study was 12.5% and were compairable to previous studies The most common cognitive enhancer use was donepezil and most common antipsychotic used was quetipine as shown previosly in a study by krishnaprasad et al
