2 Maintenance Complete and Synch all data in by 12am EST for Production on Tuesday evening, August 23rd. Valet the database any time after 5am EST on Wednesday, August 24th To prevent loss of data, do not run Valet until all visits (including Paused and Incomplete visits) have been fully completed and synched back to the office

3 Restyled Back Office The HCHB Back Office application will soon have completely redesigned screens. Here is an example of the New Referral Screen Eventually all HCHB screens will adopt a similar look and feel based on the standards introduced on the Referral screens.

4 Patient Snapshot The Patient Snapshot is a collection of patient information from various areas of the application. It provides a summary view of the information, eliminating the need to access multiple areas of the application to build a picture of the patient’s status.

7 OPEN PATIENT IN… This option allows you to quickly open up the patient’s information in another console. This is only available if you have the consoles added to your Quick Access Toolbar In order to add consoles to your Quick Access Toolbar, right click, then select ‘Add to Quick Access Toolbar”

Additional requirements by CMS to capture Flu Vaccine data for all agency workers HCHB has added additional tabs to worker profiles Go Live is October 1, 2016 Based on Immunization Status selection: If received from outside of the HHA, place is required If Declined, reason is required Additional Reason Comments section Worker Information Report will be updated to include these fields in a future release

11 Look Back Option Look Back option within PointCare allows clinicians to see previous SOC, TIF or ROC visits to confirm how the Shingles Vaccine and Advance Care Plan questions were answered Ensure accurate documentation and accurate reporting to CMS to maximize VBP %

12 VBP Reporting 2016 PPS Final Rule implements new requirements needed to collect and report information needed for the HH Value Based Purchasing Model via new web portal. Value Based Purchasing Reporting - CMS Data Elements

13 Emergent PRN Orders Emergent care after hours
Gives field clinicians ability to order and schedule PRN visits when the branch office is closed when a patient needs to be seen on an emergency basis Order type automatically approved and visit automatically scheduled to that worker Setup is at the worker level and branch level

14 Emergent PRN Orders Emergent PRN Orders are ONLY available through PointCare Emergent PRN Orders are ONLY available after branch hours Clinicians will still need to contact their office to have PRN visits scheduled out to them during regular business hours Two syncs are required to complete the request

16 Emergent PRN Orders Only PRN visit types will be available to plot on the calendar Only select days will be available to plot visits on Ex: If the nurse needs a visit for the night of the 26th, and it is after business hours, the nurse can plot a PRN visit, but only on the 26th.

17 ‘Renewless’ Schema Change
The new Renewless Schema Change update method allows PointCare users to update PointCare’s schema version without performing a Renew. This allows the user to receive updates more quickly and mitigates the need to complete and sync back all visits before updating. This allows updates that require a schema change to be delivered without requiring users to complete and sync back all open visits.

18 Deleted Visits Completed visit documentation that is deleted in HCHB will now be maintained electronically within HCHB. This will assist agencies in record keeping as it will remain accessible in the electronic record rather than relying on copies needing to be created and stored.

19 Deleted Visits Documentation of deleted visits will be available in a view-only format Only those with the assigned system functions will be able to view the deleted documentation Deleted documentation will only be available in the visit notes grid, no longer available elsewhere in the medical record Only visits that have been deleted AFTER the release will be available to view

20 Hospice Related and Unrelated Diagnosis
Additional PointCare enhancements have been added to indicate ‘Related’ and ‘Unrelated’ diagnoses in Hospice SOC visits This is a result of the Final Rule that required to delineation of the Related/ Unrelated identifier

21 The pop Up menu box in Point Care removed “T” for treat and replaced with ‘Related to Terminal Prognosis’ and ‘Unrelated to Terminal Prognosis’

22 Failed Claims Submission
In the case of a failed claim submission, the user can now easily track down the failed claim Additional Filter Criteria in Claim Management: Include Failed Claims Only Helps isolate claims that were previously attempted but failed to generate the claim

23 Pre – Claim Review Update
T2 – New field for the unique tracking number entry and modified electronic claims to display the number. Plan for Service Pack on September 13, 2016 Texas is the only Cornerstone state affected Go Live is December 1, 2016 More information and education to come…
