A guide to filling out the Athletic Packet prior to tryouts.

What do I need to try out? In order to try out for any team at PBCHS, athletes must have: A COMPLETED Athletic Packet Available online or in the main office $10 Insurance Payment Birth Certificate

Athletic Packet 101 Athletic packets are required to maintain eligibility for athletes. Athletic packets should remain stapled in page number order. No athlete can be eligible for play without this packet! Please review this presentation to ensure that your packet is completed correctly.

Page 1 – Checklist Student ID Number is REQUIRED Check to be sure all items are complete before submitting athletic packet.

$75 Insurance Fee is required for ALL athletes
Page 2 - Insurance $75 Insurance Fee is required for ALL athletes $10 Tryout Fee must be paid prior to participation $65 Balance is due upon placement on a team Note: This one-time payment covers all sports played in the same school year.

Page 6 – Physical (2 of 3) This page MUST be completed by a PHYSICIAN.
Physician MUST clear athlete to participate. Physician MUST sign & date. Physician stamp should include name, address and phone number. Physicals are valid for one calendar year.

Page 7 – Physical (3 of 3) This page is only completed if the primary physician does not clear the athlete. Athletes should be referred to a specialist for re-evaluation and clearance.
