Maneuvering through the Confusion that Surrounds Medication Monitoring

Objective Assure that we, as healthcare professionals, are monitoring medications properly and safely while in our clients home

Medications In The Home
Pills Liquids Nebulizer (breathing treatments) Injections (insulin, anti-thrombolytic therapy, etc) Topicals (creams, sprays, etc) Drops (eyedrops, ear drops, nasal drops)

Can we administer any of the items listed?
NO!!!!!!

BUT WHY NOT According to the Medicaid Scope of Services for PCII
Monitoring medication (for example, informing the client that it is time to take medication as prescribed by his, or her, physician and as written directions on the box, or bottle, indicate). The aide cannot administer the medicine; however this does not preclude the aide from handing the medicine container to the client

OR WE CAN……. REMIND HAND THEM THE PILL ORGANIZER
“Hey, Mr./Mrs. Jones its time for you to take your medication, check your blood sugar, breathing treatment, etc.” Per the Scope of Services we can hand them the pill organizer with their medications already prepared. OR

WE CANNOT Give our client medications out of their prescription bottles Give our client shots (insulin, etc.) Apply prescribed topical creams to our clients

WE CANNOT Place drops into the eyes or ears
Place the medication in the nebulizer machine (however we can assist them with placing the face mask on for the breathing treatment) Check blood sugars (however we can setup the glucometer, gather the supplies needed, open alcohol swab, etc)

In Review Medications can NOT be administered in the home by the caregiver We can assist our clients with setting up and cleaning medical equipment We can remind our clients to take medication Caregivers are able to hand the client the pill organizer BUT are NOT allowed to set it up
