“The FACE of Boone County”

2 In the beginning… On November 6, 2012, the citizens of Boone County passed County of Boone Proposition 1, which created a Children’s Services Fund for children and youth nineteen years of age or less in Boone County.

3 By statute, funds may be invested to address the following needs:
up to thirty days of temporary shelter for abused, neglected, runaway, homeless or emotionally disturbed youth respite care services unmarried parent services outpatient chemical dependency and psychiatric treatment programs counseling and related services as a part of transitional living programs home-based and community-based family intervention programs prevention programs which promote healthy lifestyles among children and youth and strengthen families crisis intervention services, inclusive of telephone hotlines individual, group, or family professional counseling and therapy services psychological evaluations mental health screenings

4 Disorganization of existing service system, and
In an effort to better understand children’s services in Boone County and make wise use of the Children’s Services Fund, the Boone County Children’s Services Board (BCCSB) authorized a project to research the needs and services in Boone County. The result was the publication of the Community Impact Report (July, 2014) This report identified three broad barriers to services and supports for the families of Boone County: Lack of access, Disorganization of existing service system, and Lack of a family systems approach to prevention and intervention services.

The FACE of Boone County was born!

6 The FACE of Boone County seeks to
specifically and directly address the three broad barriers identified in Community Impact Report (July, 2014) Access Coordination Family Centered Approach

7 By providing a multiple door referral process that leads to:
Case Management a coordinated, nonconflicted approach founded on comprehensive, evidence-based assessments, Family Choice referral to a menu of evidence- based services based on identified needs, Continued Support ongoing monitoring of the success of those referrals to ensure services are accessed by youth and families, and Accurate Measurement progress monitoring of outcomes of youth receiving services in the community. How will we do this?

Families Simplified process for identification of needs Simplified process for identification of community based supports and programs Simplified process for accessing and securing needed MH services and supports Providers Streamlined process for receipt of referrals Access to more appropriate referrals based on agency services provided Increased training and technical assistance to support improved services and outcomes for families
