Your Coverage remains an AETNA PPO There is no change in Network All services effective 1/1/2018 are subject to your Deductible Once the Deductible has been satisfied: Your cost share for Medical will be 20% of the Aetna Negotiated Rate Your cost share for Prescription Drugs will be at the Co-Pay Schedule described subsequently You do not need Referrals. Pre-Certification & Prior Authorization may be required for certain services.

To maximize your benefits, you must use Aetna Providers. Lab Work: Quest Diagnostics is your Designated Site for Lab Work You are able to receive reduced rates: At Urgent Care Centers As an alternative to Emergency Room visits At Out-Patient Surgical Centers or Free-Standing X-Ray Facilities As an alternative to Surgical Procedures or High Level X-Ray/MRI, etc. Aetna Navigator: Website allows you to use Aetna’s Cost Estimator for Services as needed. Track your Claims, Deductible Usage, Print ID cards as needed.

7 Aetna Medical & TASC HRA Reimbursement plan
Your new AETNA Medical Benefit has the following Deductibles: $5,000 for Single $10,000 for Employees with Dependents EMPLOYER has funded the initial portion of your Deductible with a TASC Health Reimbursement Account (HRA) as follows: $2,000 for Single $4,000 for Employees with Dependents Enrolled Employees will get (2) Identification Cards 1. Aetna PPO ID Card – No Referrals Required 2. TASC HRA Debit Card – Preloaded with Amounts Shown Above

8 How to use your tasc hra debit card
For Office Visits, Laboratory Tests, X-Ray Services, Therapy, In-Patient / Out- Patient Hospital Services.* (* No Referrals needed. Pre-certification and Prior Authorization may apply.) Present your Aetna ID Card to all Providers at Time of Service. In most cases you will not be required to pay at Time of Service. Provider will submit charges to Aetna. Aetna will process the claim and send you a detailed Explanation of Benefits (EOB) indicating the amount you owe the Provider, based on what was applied to your Deductible for each Date of Service. Provider will send you an Invoice with the corresponding amount for that Date of Service. Pay your Provider: Call or mail the information listed on your TASC Debit Card to the facility for payment using your card as you would an ordinary credit or debit card.

9 Pharmacy instructions (retail & mail order prescriptions)
Present your Aetna ID Card Pharmacy will charge you the Aetna Discounted Rate for each Prescription (which is then applied to your Deductible). Pay the Pharmacy (or Mail Order) using the TASC HRA card, just as you would with any other credit/debit card. Please Note: The Pharmacy is usually the only place you use the TASC HRA card at the Time of Service. The HRA card can be used at the Pharmacy for Prescription Drugs ONLY.
