Practice Integration Maine Access Health Foundation (MeHAF) Site Self Assessment (SSA) 60-90 minutes Create a team and have a facilitator You can create a practice profile Practice Integration Profile (PIP) Less than 10 minutes BHP and medical director complete Generates a practice profile for your clinic (free!) “Needs Assessment” PIP: The PIP takes about 10 minutes to complete and has two purposes. First, it is meant to help you and your practice to assess where you are with your integration efforts. Second, we will use the results to improve the survey itself. All information will be analyzed and reported in a form that does not identify you or your practice. Responding to all questions is extremely important.
