2. OPADD Begins 2005 – Regional OPADD conference in Hamilton
Brant community participates at Regional level
Interest wanes at local level

3. Issues Arise
Spring of 2008 – Two individuals with developmental disabilities admitted to two LTC homes
Both individuals create significant issues in new environments
In danger of losing placements

4. The Plan Group from LTC and DS services meet to discuss issues
Meetings productive - helped each sector to understand issues and limitations experienced in each sector
Provides avenue for discussion about issues and problem solving

5. Plan (con’t) Plans developed for each individual including:
training for LTC staff
staffing resources from DS sector for transition
assistance from DS specialized services providers

Sadly one LTC placement broke down despite everyone’s efforts

But the other placement has remained successful and we have learned a lot

8. Long Term Results
Group continues to meet to discuss other potential issues and brainstorm better ways to provide services for the future
Group continues to meet almost monthly
Added members from CCAC, local hospital discharge planning, Alzheimer’s society and the local psycho-geriatric mental health outreach program

We have determined:
to become more educated about each other’s services
to work more collaboratively
to partner, where appropriate, to provide services to this population

Proposals submitted from both sectors to enhance the services to this population
Invited LHIN rep to meeting to discuss Aging At Home Strategy initiative and to determine possible joint proposals for funding
