What’s the context?:
‘Care Together’ is the Integration of Health and Social Care in Tameside
It is a ‘whole system reform’ –that means everything!
The Care Together programme is whole system reform – that means everything – CCG, hospital, adult social care, children’s social care, public health - everything!
Not to be confused with Healthier Together – which is a GM reform of hospital services!

4. What is the current thinking?
Tier 0 services – this tier will actively build capacity/resilience within communities to reduce unhealthy behaviours and increase levels of self-care through support of health champions and peer support. Aligned to the Neighbourhood Offer, it will also increase community awareness of key messages through the delivery of local and national campaigns and promote the self-help principle for those that are able to. This tier is universally looking to deliver on the foundation programmes identified in the Joint Health and Wellbeing Strategy
Tier 1 services– this tier will provide points of access into the Wellness Offer for those that require more personalised support (telephone, web, face to face) e.g. structured support services in community settings such as Health Trainers. It will provide holistic needs assessments and deliver brief intervention advice and referrals to other lifestyle services as appropriate as well as services providing advice and guidance on wider social issues including employment, debt and housing.
Tier 2 services– this tier will provide structured interventions e.g. stop smoking, weight management etc and will be delivered in localities with strong links to primary care. The vision is that Level 2 services will utilise practitioners who are able to offer support across all lifestyle issues, people are not referred to different services but receive advice about the subject they prioritise but with the possibility of additional support for other issues as they are explored. These services will be incentivised to identify and support clearly defined priority groups as identified through risk stratification, needs assessments and evidence based commissioning.
Tier 3 and 4 services will not be covered in the Wellness Offer but through structured referrals to more complex support and treatment services

Co-ordinated approach to a VCFS ‘offer’ (the commissioners are likely to want a ‘single point of contact’)
