6. Inpatient Linkage
~20% of people admitted to hospital as inpatients have diabetes
More effective training for ward staff prioritised by Think, Check, Act which aims to:
Reduce rate of hypoglycaemic events
Increase appropriate management of hypo events
Reduce length of stay
Aim to support provide custom functionality for patient management and reporting

13. Scottish Ambulance Service
Highlight high risk patients who have required the assistance of the Scottish Ambulance Service (SAS). This will enable these patients to be identified to ensure that they receive appropriate follow-up care.
SCI-Diabetes will receive records from SAS that contain a blood glucose result, as the intention is to highlight patients with poor glycaemic control.
