Collaboration: Macmillan & BCUHB Welsh Government Cancer Delivery Plan ( ) Macmillan ‘Recovery Package’ The cancer story is changing Survivorship More people living with and beyond cancer Develop sustainable models of care A cultural shift in cancer care The Welsh Government Cancer Delivery Plan – Sets out the WG vision for the population of Wales and what this means for NHS Cancer Services. It shows the WG commitment to delivering the best possible care to everyone affected by cancer. WG CANCER DELIVERY PLAN The Macmillan Recovery Package – Is a series of interventions which when delivered together can greatly improve outcomes for people affected by cancer. Health & Wellbeing Events (An Education and Support Event) are a key component of the Recovery Package. RECOVERY PACKAGE BOOKLET Sustainable models of care – We work in a challenging healthcare funding environment, so there is a clear need for cancer services to adapt and support people to self-manage their condition, enabling better health and care outcomes. We need to develop sustainable models of care to meet the needs of people living with and beyond cancer. A cultural shift in cancer care!

4 In the UK: There are currently 2.5 million people living with and beyond cancer. That number is set to rise to 4 million by 2030. In North Wales: There are currently 27,000 people living with and beyond cancer. That number is set to rise to 48, 800 by 2030. This reflects: Although more people are being diagnosed with cancer in the UK, more people are surviving due to earlier detection and improved treatments. A growing ageing population. Increase in cancer incidence. Cancer is moving from a disease where mortality was often the norm to one where more people can expect to live healthy and active lives for many years once their treatment is over. This means...We have an enormous challenge ahead!!!

5 Living longer: People now live nearly 10 years longer after diagnosis than they did 40 years ago. Living well? Those living with and beyond cancer often have poor health and wellbeing. They may benefit from further support. Unmet needs of people include physical and psychological effects as well as financial and practical. The National Patient Reported Outcome Measures (PROMs) have found that people with cancer experience persistent long term problems, experience unmet needs at the end of treatment and a re likely to have poorer quality of life scores. Health & Wellbeing Events are intended as a means to provide such support. People living with cancer want to be more informed and involved with their own care.

6 What are Health & Wellbeing Events?
Education & Information Support people affected by cancer, carers, families, friends Take control & participate in recovery Supports transition through cancer journey Promote positive lifestyle change Bring together services: health, social, community, voluntary Main aims: Provide Health & Wellbeing information & advice Signpost to local resources & services Facilitate interaction with others affected by cancer Health & Wellbeing Events are a Macmillan Cancer Support development. They are all about education and information. They support people living with and beyond cancer, their carers and their families to take control and participate in their short and long term recovery. They give necessary information and promote positive lifestyle change. A Health & Wellbeing Event is a service model that takes the form of a supportive event to help prepare people affected by cancer, their carers and their family and friends for the transition through their cancer journey. The event brings together a range of service providers from health, social, community and voluntary sectors. A holistic offering. The main aims are: - To provide information and advice relating to health & wellbeing - To signpost those attending to local resources and services, both clinical and non-clinical - To facilitate interaction with other people affected by cancer

7 Benefits of Health & Wellbeing Events
Benefits to patients: Support self-management Better educated / Informed health decisions Improvements in wellbeing / quality of life Increased knowledge of support Benefits to professionals: Reduce reliance on health services Efficient provision of information Adapting previous provision: Evaluation & Scoping New Macmillan ‘How To’ Guide Health & Wellbeing Events offer a range of benefits to many different audiences. Benefits to patients: These are numerous, but I will just focus on a few... The events support self management. People affected by cancer achieve a greater sense of control, feel more confident and are more easily able to access the support they need when they need it. People are better educated on the signs and symptoms of cancer recurrence and the consequences of treatment. People are able to make informed decisions about their health as a result of the information that they receive and therefore may experience improved wellbeing. People can build their knowledge of local and national support services that are available to support them. They enjoy a sense of reassurance knowing how to access support when they need it. People are able to freely discuss their needs and feel less isolated. Benefits to professionals: Events allow people to manage their own health more effectively, reducing the need for support from health services. Offering information to a group of patients is more effective than multiple 1:1 consultations Effective way of ensuring that patients are supported through their cancer journey and that they continue to feel supported after treatment. 8 When are the Events taking place?
The Beaches Hotel, Prestatyn Wednesday 26th April 2017 Wednesday 27th September 2017 Theatre Clwyd, Mold Wednesday 24th May 2017 Wednesday 25th October 2017 Plas Menai, Caernarfon Wednesday 21st June 2017 Wednesday 15th November 2017 10am – 1pm (Register from 9:30am) Flexible Agenda These locations were chosen after analysing the percentage of cancer patients in all areas across North Wales. Events are held in community venues, not clinical settings. Events are spaced every 5 months apart in each area, but with an event running each month (April, May, June and September, October, November) – break July and August due to professional support.

Wellbeing Information Zone (Talks & Tasters) Ask an Expert Hub Health & Wellbeing One Stop Shop Social Area Quiet Area Refreshments A new Evaluation *Contents of the day may vary by area Registration / Event Information Pack: Feedback / Evaluation Sheet, Agenda for the day, Floor Plan of the Event, Booklet of support agencies at the event, Macmillan booklets, Bag and Pen. Wellbeing Information Zone (Talks and Tasters) including: There will be a range of mini talks and taster sessions available with information to support patients during and after cancer treatment. Aiming to include: Welcome & Intro, Managing the possible long-term affects of treatment, The importance of healthy eating, The benefits of physical activity, Fatigue management, Mini physical activity taster session, Emotional support, Mini mindfulness taster session, Macmillan CISS and Benefits / Financial Advisors. Ask an expert hub: Have a chat with the experts. A range of health experts from different backgrounds will be available on the day to answer questions in an informal environment. Hoping to include: CNS (Breast, Lung, Urology, Colorectal). Professionals who did talks: Dietician, Physiotherapist, Occupational Therapist, Psychologist, Senior Nurse and Q’s ballot box. Health & Wellbeing One Stop Shop: Get to know support available in the local area. Opportunity to speak to professionals from local support services in the community on a range of issues. Hoping to include... Physical Activity – NERS, Let’s Walk Cymru, Mens Sheds / Macmillan – CISS, Libraries, Welfare Benefits / Tenovus – Welfare Benfits, Tenovus Choir, Councelling / Social Prescribing – DEWIS, SPOA, CVSC / Wellbeing – Boots No7, Red Cross, Look Good Feel Better / Wellbeing Courses – EPP, Macmillan HOPE / Also – Community Pharmacy, NWCPF, Parabl, Care and Repair, Carers Groups ETC / Chemo / Radio DVD on repeat and others. Social area: A chance to meet others who have had a similar experience. Grab a cup of tea and a chat with someone who understands how you might be feeling. Quiet area: People feeling unwell or upset. Evaluation Drafted a new Feedback sheet for events. So that we can learn as much as possible from all people attending what is working well and what is working not so well so we can continuously develop and change to ensure that they meet they meet everyone’s needs.
