Our intention today is to tell the story of an effective and evolving partnership with faith based resources.
As healthcare institutions we are charged with prevention and the promotion of health and wellness among our population.
The Conetoe work is a shining example of results because of the extreme alignment of work and small resources. It has impact because it addresses needs that the faith based community is more capable of influencing.
Extremes in illness and chronic disease is usually more than biological. As hospitals we can address acute biologic illness, but we are not good at influencing some of the more substantive issues like social influencers.

13. What is Conetoe Family Life Center?
The Conetoe Family Life Center, established in 2007, mission is to improve the health of the youth and community by:
Increase access to healthy food
Increase physical activity
Provide access to health services
Educate and partner with youth in communities to influence families and break the cycle of poverty and hopelessness
“Change the children by education and opportunity = Change for the family.”

14. Aftershool program
Tutors help youth to learn, and then later become tutors to others
35 kids in afterschool
Graduation rate has improved from 40% to 98% of students served in the program
No unplanned teen pregnancy
We continue to work with these kids until they demonstrate a sustainable lifestyle
Teaching parenting skills
