Eastern North Carolina Demographic Trends Our intention today is to tell the story of an effective and evolving partnership with faith based resources. As healthcare institutions we are charged with prevention and the promotion of health and wellness among our population. The Conetoe work is a shining example of results because of the extreme alignment of work and small resources. It has impact because it addresses needs that the faith based community is more capable of influencing. Extremes in illness and chronic disease is usually more than biological. As hospitals we can address acute biologic illness, but we are not good at influencing some of the more substantive issues like social influencers.

Health is a state of complete physical, mental and social well-being, and not merely the absence of disease or infirmity. -World Health Organization At Vidant Health we are seeking to improve the health & wellness of our communities. Health is more than the absence of disease or infirmity.

The model of social determinants include: Education Occupation and productive work Income Social status Gender Race

Eastern NC has a disparity in health likely representing or resulting from social determinants

Environment Resources Healthcare plan compliance Church settings may be significantly more capable due to their unique influence on behaviors, the mechanism to address each of the above issues, etc. Joyner was aware and interested in correcting the “cycle of poverty” and hopelessness among the youth and families.

13 What is Conetoe Family Life Center?
The Conetoe Family Life Center, established in 2007, mission is to improve the health of the youth and community by: Increase access to healthy food Increase physical activity Provide access to health services Educate and partner with youth in communities to influence families and break the cycle of poverty and hopelessness “Change the children by education and opportunity = Change for the family.”

14 Aftershool program Tutors help youth to learn, and then later become tutors to others 35 kids in afterschool Graduation rate has improved from 40% to 98% of students served in the program No unplanned teen pregnancy We continue to work with these kids until they demonstrate a sustainable lifestyle Teaching parenting skills

15 Community Garden Social & Economic engine
Provides nutrition (feeding 62 families) Engages the community Encourages activity & exercise Teaching skills of business Develops social skills through the work

Produces honey for produce market Environmental awareness and stewardship Develop a certification and technical skills Youth are learning sales and customer service as “store reps” The

18 Bee Program Graduate Community college allowing people to get education and training as lay health coaches

100% students are engaged in after-school summer programs 98% graduation rate Human development plans for each of the children Medication compliance and rationed 100% students buy “back to school” supplies through the produce sales $24,000 scholarship available through the produce sales Increased advanced directives 80% of congregation Insert the outcomes
