Salim Khila Musallam Al-Junaibi years old Driver Married DEAD Simon Arimangalath Samuel years old Instrument Technician Married DEAD SEQUENCE OF EVENTS On Sunday 04/03/01 at 1800Hrs Maqbool and Faisal decided to leave WPH13 camp in Yibal, without permission, and go home to Muscat for the EID celebrations. They secured a lift from one of their colleagues to Nizwa and continued to Muscat by taxi. Their aim was to pick up a car (Jeep Cherokee) belonging to another colleague and return to Yibal in time for work starting at 1300Hrs on Monday 05/03/01. They left Muscat at approximately 0930 Hrs and at 1130Hrs were on the Nizwa-Izz section of the highway when they sustained a blow-out to the offside rear wheel of the vehicle. The vehicle went off the road on the right and into the ditch, they slid for approximately 40 metres then somersaulted four times coming to rest 146 metres from entering the ditch. Both pronounced dead at scene. 1

A large oilfield truck & trailer loaded with a 60 ton mudpack travels to Rima. Speed: 60 km/hr A pick-up comes from the graded road on the right. Visibility is good. What could go wrong ???? ? Nimr to Rima Blacktop, Warad junction, am Truck Pick-up IMMEDIATE CAUSE(s) 1 High speed, from investigation speed was in excess of 150KPH. 2 Failure to wear seat belts. UNDERLYING CAUSE(s) 1 Unauthorised trip. 2 Urge to be with family during Eid celebrations. 3 Late in leaving Muscat. (Not able to make 1300Hrs start time) 4 Disregard for Omani Law (Speed Limit, Wearing of Seat Belt) 5 Failure to recognise hazards 2

The pick-up with 4 man on board arrives at this junction with the blacktop and wants to turn left. The driver does NOT STOP. He does NOT see the truck on the left or he miss-judges its speed. The driver seems to be in a hurry. He was going on rest days later that day. 3

The truck driver only sees the pick-up at the last moment: too late! The truck pushes the car 60m before it stops. 4

The driver and left passenger are DEAD. The 3rd is still in very serious condition. The 4th was hurt but is now OK. The truck driver and his helper were not hurt. All had their seatbelts on. 5

What you must learn from this accident: - As a driver: DRIVE TO SURVIVE (=drive defensively) # do not rush, take your time # scan the road ahead for hazards # be prepared for unexpected moves of other drivers # STOP at STOP-Signs (follow the rules of the road) - As a passenger: STOP/Slow down a driver # when he is speeding # when he takes risks # even when he is your boss # even when you do not know each other # even when you are from different cultures # even if your not sure of yourself 7
