Commonwealth awards Grant to Harvard SPH to carry out CSI Evaluation
October 2008
Original CSI Contract Implemented in 5 sites: Second multi-payer pilot in country to kick off
March 2009
Practices produce first quality reports.

CSI Expands to 8 new sites, including the South Cty. Hosp. Patient Centered Medical Community, and Mem. Hosp. Family Care Center

CSI Adopts New Mission: To Lead the Transformation of Primary Care in Rhode Island

CSI Receives $125,000 grant from RI Foundation to provide technical assistance to practices

CSI Submits Application for Medicare Advanced Primary Care Practice Demonstration - one of 8 States selected
