1 THE OUTCOME OF PATIENTS WITH HEAD INJURY IN POLYTRAUMA PATIENTS
V. Novak, I. Stefanović, N. Stojanović, A. Kostić, M. Radisavljević, B. Jelenković, L. Berilažić Clinic of Neurosurgery Niš, CC Niš

2 INTRODUCTION Head injury is often present in polytraumatized patients.
During the period on Neurosurgical Clinic Niš 213 polytraumatized patients has been treated. Applying Glasgow Coma Score ( GCS ) all injured patients had been divided in two groups: patients with severe head injury with GCS lower than 8 and patients with mild head injury and GCS upper then 8

3 MEHANISAM OF INJURU Mehanisam of injuru GKS=>8 GKS<8
Traffic acident 67 32 Fall from height 19 29 Fight 50 16 All 136 77 The most common mechanism of injury was traffic accident (GKS> %, GKS < %), but the high percentage of minor head injuries he sustained in a fight (36.76%).

4 Intracerebral haematom
SUPSTRATE ON CT BRAIN Supstrate on CT brain GKS=>8 GKS<8 Withaut supstrat 107 Skull fracture 19 15 Contusion cerebri 5 18 Epidural haematom 1 6 Subdural haematom 9 Intracerebral haematom 22 SAH traumatica 3 7 All 136 77 Fractures of the skull is the most commonly found substrate to CT in patients with minor head injury 13.97%, while for severe injuries that were intracerebral hematoma in 28.57% of cases.

5 ASSOCIATED INJURES WITH OTHER ORGANS AND SYSTEMS
GKS=>8 GKS<8 Thorax 23 Abdomen 13 16 Extremities 92 18 Spine 5 7 Multiple systems 3 All 136 77 Minor head injury was most often associated with extremities injuries ( 67.64%), while in severe haed injuries belonged to the chest (29.87%).

6 OUTCOME Outcome GKS=>8 GKS<8 Recovery 133 37 Neurological deficit 12 Vegetative state 6 Death 3 22 All 136 77 In 3 cases with a minor head injury was observed mortality with the dominant injury of thorax and abdomen. In severe head injuries recorded a high mortality rate 28.57% and the presence of neurological deficit at 15.58% of the patients.

7 CONCLUSION Head injury is the most common result of traffic accident. For further prognosis and treatment of patients with head injury substrate on CT of the brain is essential for the further prognosis and treatment. It is usually minor head injury with associated extremity injuries, while severe head injuries associat with chest trauma. Severe head injury is important factor of the final outcome.
