This attention to self help and effective care can pay financial dividends: Effective self-help reduces health care costs. High quality, effective care reduces the need for costly additional visits. Self-coordination of care reduces costly fragmentation.

19 A Hypothetical Care for chronic illnesses now costs in excess of $1 trillion per year. A savings of only 5% percent through PHRs for persons with disabilities would pay a dividend of $50 billion each year.
