Safety of the no-flip technique and spontaneous detachment for ShangRing circumcision in boys and men: results from a randomised controlled trial
Objectives:
Assess the safety & effectiveness of a new modification to the ShangRing techniques called the no-flip method.
Evaluate if the ShangRing could detach spontaneously on its own if left for longer than 7 days.
Methodology:
Open label randomised control trial (RCT).
230 boys and men 10 years of age and above at 2 sites in Kenya.
All participants underwent the no-flip method and were randomised to ring removal on day 7 or spontaneous detachment in a 1:1 ratio.

Safety of the no-flip technique and spontaneous detachment for ShangRing circumcision in boys and men: results from a randomised controlled trial
Results:
ShangRing VMMC was successfully completed on all participants using the no-flip method and no major problems were reported during surgery including in cases of phimosis and penile adhesions.
Adverse events rates, probability of healing and participant satisfaction between the two groups were not significantly different.
Three out of four participants in the spontaneous detachment group wore the ring until it fell off on its own.
Conclusions & Recommendations:
The no-flip technique, which simplifies the procedure, was safe, well-accepted by participants and preferred by providers.
Spontaneous detachment of the ShangRing would reduce the burden of care at VMMC service delivery points.
