2. Treoil Industries Biorefinery
RRT Activation:

Type and amount of product:
Tall oil: ~25,000 gallons (threat of release.)
CERCLA containers: ~300
Cause of incident:
Poor facility management spanning decades
Date of response:
March 13 - April 7, 2017
Responsible Party:
Campbell Land Corporation
Key operational activities:
1) Removal liquid oil outside of secondary containment.
2) Removal containers of hazardous chemicals/waste posing a threat of release.
3) Remove asbestos containing material for safety of workers and nearby receptors.
Major lessons learned:
At times compliance and enforcement tools cannot compel corrective action quickly enough. We are beginning to explore ways to work with haz waste inspectors so we can support earlier intervention at these types of sites.

4. Hanford Tunnel Collapse

Soil Subsidence above tunnel

Responsible Party:
Department of Energy
Key operational activities:
A soil subsidence was identified during a routine inspection of the PUREX tunnels at the Hanford 200 East Area
DOE and Washington DOH quickly and continuously monitored and detected no radiological contamination.
Crews at the Hanford site backfilled the hole with approximately 53 truckloads of soil.
Major lessons learned:
Washington Department of Ecology issued the DOE an Administrative Order stating that one or more violations have occurred and corrective actions must be taken.

5. Corvallis Mercury A few pounds of elemental mercury

Cause of incident:
Found jar of mercury when cleaning out garage, dropped it on porch

Responsible Party:
Homeowner
Key operational activities:
Standard mercury cleanup with relocation and private property disposal. Red Cross involved. Pets were also contaminated.
Major lessons learned:
Selsun Blue Shampoo is great at removing mercury, because it has Sulphur in it!

6. River Street Warehouse Fire
RRT Activation:
Notification only
Type and amount of product:
Asbestos roofing paper. Estimated to be ~65,340 sqft of roofing paper.
Cause of incident:
Three alarm fire at the warehouse on 5/14/17 just before 10pm local time.
Date of response:
PFD on scene 5/14-17/17, EPA took over the scene at 1300 on 5/17/17.
Responsible Party:
Owner of the condemned warehouse is the responsible party. The cause of the fire is undetermined at this time.
Key operational activities:
Key OPS Activities:
Define the extent of aerial deposition of roofing paper.
Locate and recover asbestos containing debris.
Through air sampling (stationary, pumps on crews, activity based), determine the impact of the debris on public health.
Secure and Stabilize the Warehouse Pile Site; and
Collect necessary data and other information from the Warehouse Pile Site to confirm CERCLA hazardous substances and NCP (b)(2) criteria and determine estimated cost and scope of potential time critical removal action

EUL and some of planning should be imbedded with OPS, when OPS is located apart from ICP.
