Precautions Process Overview Fatality Process - The process was to manufacture steel-frame foundation (6,310mm × 3,450mm, 1.3 ton) on which aggregate separator (screen) will be mounted using H beams (250 × 250 × 12t). They manufactured two steel-frame foundations of an aggregate separator. Fatality Process The victim and one co-worker went to the site at about 08:00, April O, 2007 and they were preparing to transport two steel-frame foundations of an aggregate separator on the workplace floor to near the structure to install aggregate supply hopper located at the corner. To move the steel-frame foundation of an aggregate separator, the victim was giving hand signals on the upper part of the structure (1.8m) and the co-worker operated the crane on the floor. At about 08:30, they put steel-frame foundation of an aggregate separator vertically and the victim removed the clamp that was supporting steel-frame foundation of an aggregate separator from the upper part of the structure to install the hopper. At that time, steel-frame foundation fell towards the victim owing to partiality load and the victim was crushed between steel-frame foundation and another steel-frame structure to death. Fatality Cause Unprepared work plan of handling heavy object To handle heavy objects, you must prepare a work plan of heavy object’s type and shape, handling method and order, workplace size. However, there was no work plan. No protections against fall from steel frame structure - To handle steel-frame structure and remove clamps supporting the structure, there was fall hazard. However, they removed the clamp without protections against fall. Therefore, protections against fall or turnover of steel-frame foundation was insufficient. Precautions Thorough preparation of work plan and education To handle heavy objects such as steel-frame structure, you must prepare a work plan of safe working process considering heavy object’s type and shape, handling method and order, workplace size and topography and train workers prior to any work. Improving preparation against fall off steel frame structure and working process - To handle steel-frame structure, you must safety measures and must not remove hanging ropes until the steel-frame structure is completely fix to prevent any potential hazard to workers by structure fall or turnover.
