1 MANAGE - CARE Prof. Dr. med. habil. Peter Schwarz
Active Ageing with Type 2 Diabetes as Model for the Development and Implementation of Innovative Chronic Care Management in Europe Prof. Dr. med. habil. Peter Schwarz

2 The Facts A growing number of EU citizens suffer from diabe-tes, posing a growing health, social and economic burden in the EU This burden is mostly driven by type 2 diabetes, which is increasingly diagnosed at younger age and is leading to a rapidly growing group of ‘older peo-ple’ (65+) with diabetes Management of this growing group poses unique challenges and outcomes remain far from optimal An increased prevalence of multiple comorbidities require tailored and integrated treatment strategies Care models need to focus on how to prioritise dia-betes treatment relative to that of other comorbidi-ties and the functional status of patients. Care fragmentation is a major problem for most chronic diseases
