Welcome to Tusculum College’s 2017 Benefits Enrollment! We are happy to announce that there are very few changes in the upcoming year. Open Enrollment is the time of year when you can add or remove dependents or change the plans you are enrolled in. We will spend the next few minutes going over benefits offered effective April 1, 2017.

2 Medical Plan Highlights for 2017
Remaining with BlueCross BlueShield of TN Effective April 1, 2017 you will have three plans to choose from. All plans utilize the BCBST Network S Core, Buy-Up or High Deductible Health Plan (HDHP) Please note that Network S does not include UT Medical Center and affiliating physicians. Your Medical Coverage will remain with BlueCross BlueShield of TN. You will have a choice between three plans effective April 1, 2017, the Core Plan , the Buy-Up Plan and a High Deductible Health Plan. All three plans utilize Network S. Please note that UT Medical Center and affiliated physicians will NO longer participate in Network S.

The Buy-up plan has a calendar year deductible of $750 for an individual and $1500 Family Coverage. The Out-of-Pocket maximum is $3,500 for an Individual and $7000 for Family coverage. After meeting your deductible, BCBST will reimburse expenses at 80% until you have met your Out-of-Pocket maximum. There is a $25 copay for Primary Care Physician and Specialty Office Visits. Preventative Care is covered at 100%. Other services such as inpatient services, out patient services and emergency room are considered at 80% after your deductible. You have a $10 copay for generic prescriptions, $25 for Preferred Brand Prescriptions and $50 for Non-Preferred Prescriptions. Specialty prescription drugs have a 30% copay. The Core plan has a calendar year deductible of $1,250 for an individual and $2500 Family Coverage. The Out-of-Pocket maximum is $4000 for an Individual and $8000 for Family coverage. After meeting your deductible, BCBST will reimburse expenses at 70% until you have met your Out-of-Pocket maximum. There is a $35 copay for Primary Care Physician and Specialty Office Visits. Preventative Care is covered at 100%. Other services such as inpatient services, out patient services and emergency room are considered at 70% after your deductible. You have a $10 copay for generic prescriptions, $35 for Preferred Brand Prescriptions and $60 for Non-Preferred Prescriptions. Specialty prescription drugs have a 30% copay. The High Deductible plan has a calendar year deductible of $6,550 for an individual and $13,100 Family Coverage. The Out-of-Pocket maximum is $6,550 for an Individual and $13,100 for Family coverage. After meeting your deductible and out of pocket BCBST will reimburse expenses at 100%. All services are subject to the deductible. Preventative Care is covered at 100%. You have a $5 copay for generic prescriptions, $25 for Preferred Brand Prescriptions and $50 for Non-Preferred Prescriptions. These copays are for a select group of preventative prescription drugs only. Please refer to the list provided by BCBST to determine if your medications are subject to the copays. If not, the cost will apply towards your deductible and out of pocket expense.

5 Where to find information
INTERNET: Physician Directory Pharmacy Information Medical Information By Phone: By visiting the BlueCross website at you may find additional information about your medical plan. For example, the BlueCross website allows you to find physicians in your network, obtain pharmacy information and view medical plan information. BlueCross BlueShield participating team members may register on the BCBST website. Once registered, you can access your claims history, print off an explanation of benefits and view how much of your deductible and out-of-pocket have been met. BlueCross is also accessible directly by phone at

6 Teladoc For those employees participating in the medical plan, we are pleased to offer Teladoc at no additional charge! Should you not be enrolled in the medical plan, you may still elect to participate in Teladoc for $5 per month Teladoc is 24/7/365 access to a doctor by phone when your primary care physician is not available, and you have a condition when you would consider using urgent care. Teladoc is most frequently used to treat conditions like cold and flu symptoms, allergies, bronchitis, urinary tract infections, respiratory infections, sinus problems and more. All Teladoc doctors are practicing primary care physicians, pediatricians, and family medicine physicians who average 15 years experience and are U.S. board-certified and licensed in your state. So… you can “see” a doctor anytime for no charge. A short video will play next further explaining how you can utilize Teladoc.

7 Teladoc Set up your account (Primary Member) Set up your dependents
Talk to a doctor anytime, anywhere Set up your account (Primary Member) Visit the website and click "Set up account". Follow the online instructions to provide the necessary information and to complete your medical history. Set up your dependents Log into your account and click "My Family" from the top menu, then select “My Dependents.” Click “Add New Dependent” and begin adding all of your dependent’s first and last name. For Minor Dependents (17 and Younger): In addition to their name, you may complete minor dependent’s medical history as well. For Adult Dependents (18 and Older): Adult dependents must enter their own medical history. They may do so by setting up their own account on and by clicking "Set up account." They should follow the online instructions to provide the necessary information and to complete their medical history. Request a Consult Once your account is set up, request a consult anytime and anywhere you need care. With your consent, Teladoc is happy to provide information about your Teladoc consult to your primary care physician. If you have already set up your account with Teladoc, you may wish to update your medical records and well as your dependents’ medical records. If you’re new to Teladoc, follow these simple steps to set up your account. First, you’ll set up your own account. Simply visit and click "Set up account". Follow the online instructions to provide the necessary information and to complete your medical history. Second, you may set up your dependents. Log into your account and click "My Family" from the top menu, then select “My Dependents.” Click “Add New Dependent” and begin adding all of your dependent’s first and last name. For those dependents 17 and younger, you may also add their medical history information. For adult dependents (those 18 and older) they will need to complete their own information. They will log into Teladoc.com and click "Set up account". They should follow the online instructions to provide the necessary information and to complete their medical history. Now you’re ready to request a consult! You may request a consult online at Teladoc.com, through the mobile app (my personal favorite), or by calling Teladoc. Coverage will continue through Guardian effective April 1, 2017 Utilizes the Dental Guard Preferred Network You may use any dentist that you choose If you select an in network dentist you will see significant savings and discounts. You have a $50 individual and $150 family deductible. You have a maximum calendar year benefit of $1,000 plus Maximum rollover. Preventative care is covered at 100%. Basic services are covered at 80% after the deductible and major services are covered at 50% after the deductible. If you choose to utilize a preferred dentist in the Dental Guard Preferred Network, you will see significant savings and discounts. You will have a reduced deductible of $25 individual and $75 family. You will also save in out of pocket expenses as a result of the PPO discounts that have been negotiated by Guardian.

A solution to reducing cost and allowing you to get more out of your dental funds Guardian will roll over a portion of your unused annual dental maximum into an account Funds can be used in future years if the plan’s annual maximum is reached More money is rolled over if in-network dentists are used exclusively during the benefit year Maximum rollover is a solution to reducing cost and allowing you to get more out of your dental funds Guardian will roll over a portion of your unused annual dental maximum into a personal Maximum Rollover Account, which can be used in future years if the plan’s maximum is reached. As an added advantage, more money is rolled over if in-network dentists are used exclusively during the benefit year

11 2017 Dental Plan Benefits How Maximum Rollover Works

If throughout the year your benefits paid do not exceed $500, you will be eligible for a rollover amount. You will receive $250 in rollover if you use an out of network provider. If you use an in network provider exclusively, you will receive $350 in rollover. These additional dollars can be used in future years. The maximum rollover account cannot exceed $1,000.

12 2017 Dental Plan Benefits How Maximum Rollover Works
Here’s how the benefits work YEAR ONE: Jane starts with a $1,000 Plan Annual Maximum. She submits $150 in dental claims. Since she did not exceed the $500 Threshold, she receives a $250 rollover that will be applied to Year Two. YEAR TWO: Jane now has an increased Plan Annual Maximum of $1,250. This year, she submits $250 in claims and receives an additional $250 rollover added to her Plan Annual Maximum. YEAR THREE: Jane now has an increased Plan Annual Maximum of $1,500. This year, she submits $1,300 in claims. All claims are paid due to the Maximum Rollover Amount accumulated. YEAR FOUR: Jane’s Plan Annual Maximum is $1,200 ($1,000 Plan Annual Maximum + $200 remaining Maximum Rollover Amount accumulated).

Your materials are covered at 100% after a $20 copay. You may select either a pair of lenses or contacts each year but not both. Contacts are covered at 100% up to $150 if you utilize an in-network provider Your frames are covered at $40 wholesale for in network and $40 Retail for out of network. As you will see there are benefits for out of network providers however, you received much better benefits if you use an in network provider.

16 Life & AD&D: Guardian Basic Life and Accidental Death and Dismemberment Insurance is provided to all full time employees in the amount of $20,000. AD&D insurance pays an additional percentage of the amount of your life insurance benefit based on a specific list of loses such as loss of life, limb or sight due to an accident. Your benefit amounts are subject to age reductions beginning at age 65. Please see your plan document for details. Remember to keep your beneficiary information up-to-date for your company-provided Life and AD&D Insurance. This can be updated at any time throughout the year. All full time employees are eligible to purchase Voluntary Life and AD&D coverage You may select in $10,000 increments up to a maximum of five times your annual salary, or $300,000, whichever is less Guaranteed Issue of $150,000 if selected at the time you are first eligible All full time employees are eligible to purchase Voluntary Life and AD&D coverage You may select in $10,000 increments up to a maximum of five times your annual salary, or $300,000, whichever is less You have a Guaranteed Issue amount of $150,000 if selected at the time you are first eligible. This allows you to select up to $150,00 without answering any medical questions.

18 Voluntary Life & AD&D: Guardian

19 Voluntary Short Term Disability: Guardian - New
Offer Voluntary Short Term Disability coverage effective April 1, 2017 Benefits will begin of the 8th day of a sickness or accident Benefits will payable for 26 weeks You may choose any amount in increments of $50 up to a maximum of 60% of your pre-disability monthly earnings not to exceed $1,000 Tusculum College will be offering Voluntary Short Term Disability coverage effective April 1, Short Term Disability coverage can protect your income in the event of a short -term injury or illness. You disability benefits begin on the 8th day that you have been disabled and is payable for a maximum of 26 weeks. Once you have met this elimination period, you are eligible to receive 60% of your pre-disability monthly earnings not to exceed $1,000.

20 Long Term Disability: Guardian
Long Term Disability coverage is provided to you at no cost Benefits begin after you have been disabled for 180 days You are eligible for 60% of your pre-disability monthly earnings not to exceed $5,000 Your benefit duration is subject to reductions beginning at age 60. Please see your plan document for details. Tusculum College also provides to you Long Term Disability Benefits. Long Term Disability coverage can protect your income in the event of a long-term injury or illness. You disability benefits begin after you have been disabled for 180 days. Once you have met this elimination period, you are eligible to receive 60% of your pre-disability monthly earnings not to exceed $5,000. Your benefit duration is subject to reduction in the event you become disables at the age of 60 or older. Please see your plan document for details.

21 Additional Voluntary Products AFLAC
Tusculum College allows access to voluntary benefits through AFLAC Accident Insurance Covers you 24/7 for injuries on or off the job Short Term Disability No longer available. However, you may keep your current coverage and continue the payroll deduction. Cancer Insurance First occurrence benefit from $2,500 to $10,000 Specified Event First occurrence of $5,000 paid upon diagnosis of specified diagnosis Tusculum College allows access to voluntary benefits through AFLAC They offer an Accident Policy that covers you 24/7 for injuries on or off the job Short Term Disability that pays up to $3,000 based on your income Cancer Insurance that pays a first occurrence benefit from $2,500 to $10,000 and a Specified Event policy that pays a first occurrence of $5,000 paid upon diagnosis of specified diagnosis

22 Flexible Spending Account (FSA)
Who’s eligible for FSA? All full-time employees What is it? An account that allows you to put aside money on a pre-tax basis to pay for qualified medical, dental and vision expenses (health FSA) How does it work? Your contributions are payroll deducted. When an expense is incurred, you simply use your Flex debit card or request reimbursement. Health FSA money is available on day one of the plan year. You can contribute between $480 up to $1,500 to this account per year All full-time Tusculum College Employees may participate in the Flexible Spending Account or FSA. A Health FSA allows you to put money aside on a pre-tax basis to pay for qualified medical, dental and vision expenses. Elected Health FSA funds are available on the first day of the plan year. An independent registered investment advisory firm Provides personalized advise regarding investments to the TIAA retirement accounts Provide a confidential financial analysis to include a written report of asset allocation recommendations at no charge to employees Employees may retain Millennium on a fee basis to manage their TIAA accounts and any outside assets Millennium Advisory Services is an independent registered investment advisory firm. Work-LifeMatters is an Employee Assistance Program that is offered by Guardian.
