6 Electronic Service Delivery Reporting Template (e-SDRT)

7 e-SDRT - Contexte Under Health Canada’s First Nations and Inuit Home and Community Care Program (FNIHCCP), communities and Tribal Councils are currently required to fulfill mandatory reporting requirements. They are collecting and uploading client service delivery information to the electronic service delivery reporting template (e-SDRT) internal databases. In use for over 10 years, these legacy databases have become unstable and is unsustainable. That is why, Health Canada’s headquarter is looking to a replacement solution for data collection.

8 e-SDRT - Objectives Replace e-SDRT with I-CLSC for the Quebec region;
Facilitate compliance with reporting requirements; Facilitate service provision planning; Facilitate continuity of services with the province; Facilitate coordination of services offered to clients.

9 e-SDRT – The indicators
The current reporting requirements and indicators (data to provide) were defined at the time of program inception in 1999 and have never been under review. Prior to implementation of the next reporting system, a review of the program reporting requirements is proposed.

12 IT Solution - Integrated Services Network for the Adult Persons (ISNAP)
DESCRIPTION This solution provides the stakeholders with a variety of computerized tools, such as the Multiclientele Assessment Tool (MCAT), to support the clinical assessment process of the adult person. This helps to improve the services provided to clients with decreasing independence by the availability and the timely access of quality and up-to-date information on the clients. It facilitates the communication and the concertation among the stakeholders involved in the client’s care and services provision. Finally, reports from which to extract management data for supporting the comprehensive planning of services for the adult clients.
