Articles or Certificate of Incorporation/Filing Receipt By-Laws and Operating Agreements Copy of all third party agreements (equipment rental, purchase agreements, etc.) Copy of any employment (Union Affiliation) agreements Copy of any licenses Copy of Federal Business Taxes for 2016 (signed by preparer and owner of company) Copy of Federal Business Taxes for 2015 (signed by preparer and owner of company) Copy of Federal Business Taxes for 2014 (signed by preparer and owner of company) Copy of stock certificates, front and back of used certificates and the next unused certificate Copy of Stock Ledger Current Financial Statement and/or Profit & Loss Balance Sheet (2011) Lease Agreement/Deed or Mortgage Statement if Owner (Office Space) Letter from bank identifying persons authorized to conduct transactions Minutes of First Corporation Organizational Meeting and Amendments Valid Proof of Ethnicity (i.e., birth or U.S. Passport) Valid Proof of Permanent resident alien status (i.e., permanent resident card) Proof of sources of capitalization/investments Valid Proof of U.S. Citizenship (i.e. Birth Certificate or U.S. Passport) Resumes Vehicle Registration (For Business If Any) Any M/W/LBE Certification, decertification or denial of certification documentation

8 What is the Difference Between Qualifying & Certifying?
Qualification determines the eligibility of a firm to bid on and perform work for the SCA. Certification is the process for confirming that a firm is a minority-owned, woman-owned and/or local business enterprise in order to participate in the SCA’s business development programs. Firms that apply for certification must apply for qualification at the same time.

9 Benefits of Certifying with the SCA
MWLBE-certified firms can participate in the Mentor Program, which provide firms with a flexible format to develop and grow their companies within the construction industry. To participate in the Mentor Program, a firm must meet all of the following criteria: Be prequalified to do business with the SCA. Be certified as MWLBE Because the Mentor Program is targeted to the small business community, firms may not: Have average gross sales of more than $2.1 million in the last three calendar years. Bonding capacity > $1 million Access to training, bonding and capital through the SCA’s mentor program. MWLBE subcontracting goals - All hard bid contracts > $1 million (Capacity and Capital Improvement projects) are subject to a minimum 20% MWLBE participation goal. MWBE consultant criteria - The SCA incorporates MWBE participation as a selection criterion in most consultant RFPs.
